Sec.13.04 RIGHTS OF SUBJECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall
be informed of: (a) the purpose and intended use of the requested data within the collecting'state agency, political subdivision,or statewide system;
(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the identity of other persons or entities authorized by state or federal law to receive the data. This requirement shall
not apply when an individual is asked to supply investigative data, pursuant to section 13.82, subdivision 5, to a law enforcement officer.
The commissioner of revenue may place the notice required under this subdivision in the individual income tax or property tax refund
instructions instead of on those forms.
Subd. 3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject
of stored data on individuals,and whether it is classified as public, private or confidential. Upon his further request,an individual who is the subject
of stored private or public data on individuals shall be shown the data without any charge to him and, if he desires, shall be informed of the content
and meaning of that data. After an individual has been shown the private data and informed of its meaning, the data need not be disclosed to him for
six months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created.
The responsible authority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority
may require the requesting person to pay the actual costs of making, certifying, and compiling the copies.
The responsible authority shall comply immediately, if possible, with any request made pursuant to this subdivision,or within five days of
the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request
within that time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private
data concerning himself. To exercise this right,an individual shall notify in writing the responsible authority describing the nature of the disagreement.
The responsible authority shall within 30 days either: (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data, including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data
in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating to
contested cases.

Structural Notes: nMONO
General
1. All footings shall bear on natural soils or engineered fill capable of safely supporting a uniform
load of at least 2000 pounds per square foot,without harmful settlement.
2. Notify the Engineer immediately if any questionable conditions are encountered during
excavation. Footings are subject to change depending on the conditions encountered.
3. Water shall not be permitted to pond in the footing excavation. Keep bottom of
excavation dry.
4. Material for backfilling must be clean,free from wood scraps and organic material. Material to
be well compacted.
5. Provide a minimum of 6"of compacted granular material below all slabs on grade.
Reinforcing
6. All reinforcing laps shall be Class B unless noted.
7. Reinforce all wall footings with a minimum two#4 bars continuous,unless noted
otherwise.
8. Provide the following concrete cover from edge of concrete to reinforcing bars:
3"at bottom of footing.
2"at earth side of walls in contact with the earth.
1 1/2"at bottom of slabs over earth or engineered fill.
9. Reinforce all earth supported slabs with 6"x 6" -W 1.4/W 1.4 welded wire fabric unless noted.
Lap all wire fabric 6"at sides and ends.
10. Reinforcing in bond beams to be continuous at comers,or use V-6"x V-6" comer bars.
11. All continuous footings are to be centered under walls unless noted. Refer to Plans to verify all
dimensions.

12. Shop fabricate all columns and beam connections to prepare for bolted field connections.
13. Use 3/4"diameter A-325 high strength bolts at all steel to steel connections.
14. Column top plates to be 1/2"thick.
15. All welding to be performed by qualified AWS operators.
Masonry
16. Reinforce masonry walls as shown on the plans plus joint reinforcing at 16"on center unless
noted otherwise.
17. Reinforce all bond beams at precast plank bearing with two#4 bars continuous,fill with 3000 psi
concrete.

18. Use a minimum of two studs to frame or support headers at all opening jambs and below
Microlam beam bearing locations unless noted otherwise.
19. Roof truss framing and floor truss framing is shown as a general layout only. Consider truss
spacing as the maximum as shown on the plans.
20. Install metal framing anchors at all connections between roof trusses and their bearing walls.
Provide metal truss hangers at all joists and trusses which bear into the side of a supporting
member.
21. At all exterior walls bolt the wall sill plate to the masonry using 1/2"diameter anchor bolts
spaced at 6'-0"on center(maximum).
22. Structural Roof Sheathing shall be 5/8" -32/16 APA Rated Sheathing with exterior glue at all
flat roof areas. The plywood panels shall be fastened in accordance with the recommendations
of the American Plywood Association(APA).
23. The Structural Floor Sheathing at mezzanine and flat roof areas shall be 3/4"T&G 48/24 APA
Rated Sheathing with exterior glue. The panels shall be glued and nailed to the framing
members as recommended by the APA.
24. All prefabricated wood trusses shall be manufactured in accordance with designs prepared by a
Registered Professional Engineer. Submit shop drawings for each type of truss showing bearing
locations,truss lengths,member sizes and grade of lumber,member stresses, connecting plate
sizes and material,and bridging locations. The shop drawings shall be certified by an Engineer
registered in the State of Minnesota.
Design Live Loads
The Structural design is based on the following superimposed Live Loads:
Roof(Snow)- 40 PSF(plus drifting loads)
Floors - 40 PSF at living spaces
50 PSF at garage floor
Wind - 80 MPH
The Structural design is based on the building in the completed state. Contractors shall take the necessary
precautions to insure that the building is braced during construction to resist all vertical and horizontal
loads encountered.
