AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed according to
the approved plans and specifications,applicable City approvals,and the
State Building Code. This permit is for only the work described and does
not grant permission for additional or related work which requires separate
permits. All provisions of laws and ordinances governing this type of work
shall be compied with whether or not specified herein.This permit will
expire and become null and void if construction authorized is not
commenced within 180 days of the date of issuance,or if construction is
suspended for a period of 180 days at any[ime afrer work has commenced.

Prior to the issuance of a Certificate of Occupancy an as-built survey and hardcover calculations
must be submitted and approved. . �
In the event of winter or other extended unfavorable weather conditions (which prevent the
completion of the exterior improvements and/or as-built survey) a Temporary Certificate of
Occupancy(TCO) may be necessary. A TCO requires a $10,000 escrow.
Advisory Comments
Any changes to the exterior/landscaping improvements, i.e. patios,grading, sidewalks, retaining
walls, etc. not currently shown on the approved survey and landscaping plan will require a �
separate Zoning Permit application to be submitted and approved prior to the work
commencing. • "
Any retaining walls that are over 4-feet in height or tiered walls not separated by twice of the
height of the lower wall require engineered plans and a building permit to be submitted and ��
approved prior to construction.

We received a demolition permit application for 2040 Spates Avenue. I've reviewed and sent the builder an email with
the additional information needed.
We also received a building permit application for a new single family home. Adam has a copy of the building plans,
survey and erosion control plans. Roger has the same, plus the entire file.
Observations during my review:
• Need to show a sidewalk connecting front door(south side)to the driveway
• Need landscape plan
• Need top of foundation location shown on the perimeter of the foundation
• Need MCWD permit
• Need escrow agreement
• Survey shows two window wells; building plans only show one

In conjunction with the demolition permit application, please have the property owner review, sign and return the
attached Zoning Disclosure Form. Also attached is an escrow agreement combining the demolition and building permit
applications. Please have the property owner sign and return the escrow agreement with a check for$2,500.
We also need:
• Documentation from the State pertaining to the well (i.e. is it being abandoned, capped, etc.)
• A survey, aerial photo or sketch plan showing all structures on the property; noting which structures are to be
demolished.
• A survey, aerial photo or sketch plan showing proposed erosion control measures for the demolition.
• Submit a copy of permit approval from the Minnehaha Creek Watershed District (MCWD). The City will not
issue a demolition permit without a copy of the permit from the MCWD or documentation stating your project
does not trigger their permitting requirements.

On July 19, 2016 the City received a building permit application for a new single family home. Staff conducted a
preliminary review based on the information provided and requests the following items be submitted or revised in
order for your application to be considered complete and for the plan review to continue:
1. Certificate of Survey. Our engineer has reviewed the survey submitted and has the following comments:
a. The survey shows two window wells,but the building plans only show one. Please clarify.
b. Show a sidewalk from the front door(south side)to the driveway(see#2, Hardcover below).
c. Top of Foundation. The top of foundation elevation is shown on the survey. Please have the surveyor
show the point or spot where the top of foundation elevation is in reference to. Please note,we expect
the location to be consistent when submitting the foundation as-built survey.
2. Hardcover. Per City Code Section 78-1683,copy enclosed,there must be a sidewalk connecting the front door
(south side)to the driveway. Please update the hardcover calculations and survey to reflect the addition.
3. landscape Plan. Prior to the issuance of the building permit a landscape plan must be submitted showing all
the proposed exterior/landscaping improvements,i.e.patios,grading,sidewalks,retaining walls,etc. The plan
should include the name of the individual performing the work. Any proposed patios, grading, sidewalks,
retaining walls shown on the landscape plan should also be reflected on the survey.
4. Separate City Permits Required for:

The following hardcover items shall be included in proposed hardcover calculations
regardless of whether the noted features are proposed to be constructed at the time of a building
permit application:
(1) Proof of a two-car garage (detached or attached);
(2) A driveway for all garages, a minimum of 12 feet in width, subject to the standards in
section 78-1681:
(3) A 24-inch wide sidewalk connecting the front door to the driveway;
(4) The minimum stairway or landing at all exterior doors as required by the building code;
and

A site inspection and monitoring by District staff will be pertormed where the activity invalves:
• a commercial/industrial/mul#i-family residential development
• a single family residential develapment greater than 5 acres or of any size if within the �
Minnehaha Creek subwatershed
• any alteration of a floodplain or wetland
• dredging within the beds, banks or shores of any protected water ar wetland
• a violation �
� any project which in the judgment of the District staff should be inspected due t� project
location, scope, or construction techniques
In threse cases, the applicant shall pay to the District a fee equal to the actual costs of field
inspec#ion of the work, including investigation of the area afifected by the work, analysis of the
work, and any sutasequent monitaring of the work, which in the case of a violation shall be at
least$35.

2. Temporary ground cover shall be MnDOT seed mix 150 shall be
applied at 100 lbs/ac. MnDOT Type 1 mulch shall be applied
at 2 tons/ac and disc anchored in areas not covered by sod
or erosion blanket.
3. Fertilizer shall be MnDOT Type 1 10-10-20 and applied at 200
lbs/ac. Disc fertilizer into top 3" of soil. Specification
reference is MnDOT 2575.
4. Dormant seed mix shall be used after November 1 or when
temperatures do not exceed 40° F, using some rates specified
above. No seed shall be placed on snow or ice greater than
2" in depth.
5. Any seeded areas that do not become established with
vegetation shall be reseeded at Contractor's expense.
6. Erosion blanket shall be installed in seed areas with ground
surface slopes of 5H:1 V or steeper.
