You were notified of the City's foundation as-built and final as-built survey requirements at the time of the
building permit issuance for the home permit. The permit is attached. The survey requirements were clearly
outlined on the building permit and were signed/initialed by the person to whom the permit was issued
indicating acknowledgement of the survey requirements. We spoke on the phone on June 17 & July 1
regarding the pre-framing requirement to submit a foundation as-built survey for the project at 4300 Sixth Ave.
To date, we still have not received the required survey and construction activity continues on the site.
You were given a deadline of July 8 to submit the required foundation as-built survey. Please be advised that
this email is your final notification if we do not receive the required a foundation survey before NOON
today a Stop Work order will be posted at the site and all workers will be sent home until this is
resolved. Your cooperation in this matter is appreciated.

Christine,
I performed another site inspection regarding the Asbuilt survey for 4300 6`h Avenue today. The site has been
permanently stabilized and vegetation is adequate. For your information, it appears that the homeowner added a little
height to the berm on the west property line as requested by the adjacent property owner, even though we had
approved the swale as shown on the Asbuilt survey. This should help ensure runoff is not directed onto the adjacent
property and relieve the concerns of the neighbor. If you have any questions or comments, please contact me to
discuss.
