The permit is attached. The survey requirements were clearly <br /> outlined on the building permit and were signed/initialed by the person to whom the permit was issued <br /> indicating acknowledgement of the survey requirements. We spoke on the phone on June 17 & July 1 <br /> regarding the pre-framing requirement to submit a foundation as-built survey for the project at 4300 Sixth Ave. <br /> To date, we still have not received the required survey and construction activity continues on the site. <br /> You were given a deadline of July 8 to submit the required foundation as-built survey. Please be advised that <br /> this email is your final notification if we do not receive the required a foundation survey before NOON <br /> today a Stop Work order will be posted at the site and all workers will be sent home until this is <br /> resolved.
