According to City Code Chapter 31, operating a massage therapy center within the City of Orono
requires a background check and current massage therapy license issued by the City. On
March 18, 2011 you were sent a Massage Therapy License Application based on your intent to conduct
massage therapy at the above address. The April 15, 2011 deadline to return the completed
application and fees has since passed and as of today, we have not received your completed massage
therapy license application.
We have noted different media advertisements promoting Seidou Salon Spa's massage therapy
services. This letter is your notice that you are not authorized by the City to conduct massage therap�r
services and all massage therapy activity must cease until a completed application and fees are
received ($100 annual license fee and $150 background check fee), background checks are
completed, and the license is issued by the City Clerk.
Please complete the enclosed applications and return to me, along with payment of the $250 fee by
Wednesday, May 4, 2011. The business will be inspected on or after May 4, 2011 for compliance.
