Sec.13.04 RIGHTS OF SUBJECTS OF DATA � .
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Inforcnation required to be given individual. An individual asked ro supply private or co�dential data concerning himself
shail be informed of: (a)the purpose and intended use of the requested data within the collecting state agency,polipcal subdivision,or statewide
system;(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or
refusing to supply private or wnfidential data;and(d)the idendry of other persons or entides authorized by state or federal law to receive the data.
This requirement shall not apply when an individual is asked to supply investigative data, pursuant to secrion 13.82, subdivision 5, to a law
enforcement officer.
The commissioner of revenue mav nlace the notice required under this subdivision in the individual income tax or oronertv tax refund
instructions instead of on those fortns.
Subd. 3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be informed whether he is the
subject of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who
is the subject of stored private or public data on individuals shall be shown the data without any charge to hirn and,if he desires,shall be informed
of the con[ent and meaning of that data. Afrec an individual has been shown the private data and informed of its meaning,the data need not be
disclosed to him for six months thereafter unless a dispute or action pursuant to tltis section is pending or additional data on the individual has been
collected or created. The responsible authority shall provide copies of the private or pubiic data upon request by the individual subject of the data.
The responsible authority may require the requesting person to pay the actual costs of making,certifying,and compiling the copies.
The responsible authority sha11 comply immediately,if possible,with any request made pursuant to this subdivision,or within five days
of the date of the request,excluding Saturdays, Sundays and legal holidays, if immediate compliance is not possible. If he cannot comply with
the request within that time, he shall so inform the individual,and may have an additional five days within which to comply with the request,
excluding Samrdays, Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individuai may contest the accuracy or completeness of public or
private data concerning hunself. To exercise this right,an individual shall notify in writing the responsible authority describing the nature of the
disagreement. The responsible authority shall within 30 days either. (a)correct the data found ro be inaccurate or incomplete and attempt to noUfy
past recipients of inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data
to be correct. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The deternvnaaon of the responsible authoriry may be appealed pursuant to the provisions of the administrative procedure act relating
to contested cases.

I ;. DEMOLITION
A. Verify with owner extent of existing demolition including removal of door
finishes to install new the and hardwood floors in areas indicated on plan
and removal cf acoustical the ceiling.
ii Where removal of walls, etc., occurs, adjoining surfaces shall be patched
and prepared for new finish application.
if WAI LS:
2.1. Install new walls as shown on the plans and described below:
A. New exterior walls of toilet roams are to be 10" concrete block to match
existing. Refer to structural engineering specification for design of new
exterior walls. Interior face of concrete block walls is have 2s metal firring
channels with 2" rigid insulation and 5/8" type X gypsum board, taped,
sanded and painted or to receive finish as specified.
B. New walls are to be constructed of 3 5/8 steel studs at 16 o.c. (or gauge and
size as recommended by manufacturer for wall height) with 5/8" type X
gypsum board each side. Walls are to be taped, sanded and painted or
receive finish as specified. Insulate interior walls surrounding toilet rooms
full thickness with acoustic glass fiber batt insulation.
III DOORS AND WINDOWS
3.1. New doors are to be 1 3/4" thick, flush panel, solid core, plain sliced, oak veneer.
3.2. Door frames are to be hollow metal with welded corners.
3.3. FINISH HARDWARE
A, interior door locks are to be lever handle D Series by Schlage or equal.
Finish is to be selected by owner. Ins4all kick plates on restroom doors.
IV FINISHES
FLOORS
4.1. TILE
A. Install porcelain tile in area surrounding bar and on floors of toilet rooms.
Wall file in toilet rooms is to be 4 1/4" x 4 1/4" glazed ceramic tile. Tile is
to be selected by owner.
4.2. WOOD FLOORING
A. Install wood flooring in bar and restaurant areas. Contractor is to consult
with owner about extent of new flooring.
