AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed according to
the approved plans and specifications,applicable Ciry approvals,and the
State Building Code. This permi[is for only the work described and dces
not grant permission for additional or related work which requires separate
permits. All provisions of laws and ordinances governing this type of work
shall be compied with whether or not specified herein.This permit will
expire and become null and void if conskvction authorized is not
commenced within 180 days of the date of issuance,or if construction is
suspended for a period of 180 days at any time after work has commenced.
The applicant is responsible for assuring all required inspections are
requested in confortnance with the State Building Code.This permit may be
revoked at any time for due cause.

Dear Building Department,
The intent of this letter is to comment on the design of the remodel of the Dayton residence. The scope
of this project consists of remodeling of exterior and interior walls, potentially bearing walls, which have
unknown support spans prior to demolition of the interior finishes. I will be available to address any
structurally engineering needs for modification of existing bearing walls, and/or addition of new beams
or columns. These elements of the home will be structurally designed to resist the superimposed loads
caused by wind, rain, snow, and live load as defined by Minnesota State Building Code.
I will also be available during the construction of this work should my services be needed to assist the
construction team.
The structural work and review which we will perform will be limited to the structural framing directly
associated with the structure of the residence. No other areas of the home were investigated or
reviewed for code compliance or structural integrity. Waterproofing, materials and finishes and all other
non-structural items are by others and are not within the scope of VAA, LLC.
