PERMANENT DOCK PERMIT APPLICATION
In accordance with Section 3.032 of the LMCD Code, the following application for
a Permanent Dock Permit shall be completed in all respects and submitted to the
District Office, 402 East Lake Street, Wayzata, Minnesota 55391. No person may
locate, construct, install or maintain a new permanent dock on the Lake without
first securing a permit therefor from the Executive Director pursuant to said
Section, provided that no such permit shall be required of docks requiring a
license under Section 3.08 of said LMCD Code.
A new permit is required whenever the size, type, location, or configuration of
the dock is changed. No new permit is required for repair or replacement of a
previously permitted dock where such repair or replacement does not alter the
size, type, location, or configuration of the dock. Permanent dock permits shall
not be granted where deicing equipment is needed.

The permittee shall comply cvith all rules, reyulations, requirements, or standards
of the t•linnesota Pollution Control �gency and other applicable federal, state, or

local agencies. This permit does not ooviate any requi:ement for federal assent
from the U.S. Army Corps of Engineers, 1135 U.S. Post Oifice & Custom House, St.

Paul, Diinnesota 55101.
IX. After completion of the project authorized, permittee s1a11 notify the Regional
," Waters' office so that a final inspection can be cond�ctzd and the permittee shall
supply the Regional Waters' office with photographs of the project area�

possible, such material should not be placed anywhere within areas subject to
floodinge In the everct spoil mus* be placed within flood plain areas because
areas free from floodin5 are not reauily available, the spoil should be placed
parallel to the direction of flood flow and/or spread over a large area so as to

shall advise the Department of Natural Resources in writing of the volume of
