Cote,20129 Wolverine Street NW,Elk River, Minnesota 55330 Plans Dated: Date Received: December 17,2009 Date Reviewed: December 30, 2009 SCOPE: This review is limited to the design of this particular project only insofar as the provisions of the Minnesota Plumbing Code, as amended, apply, and does not cover the water supply or sewerage system to which this plumbing system is connected. The review is based upon the supposition that the data on which the design is based are correct, and that necessary legal authority has been obtained to construct the project. The responsibility for the design of structural features and the efficiency of equipment must be taken by the project designer. Approval is contingent upon satisfactory disposition of any requirements included in this report. Special care should be taken to insure that the material and installation of the plumbing system are in accordance with the provisions of the Minnesota Plumbing Code. A copy of the approved plans and specifications should be retained at the project location for fnture reference. A set of the identified plans and specifications is being returned to Center Point Plumbing LLC. INSPECTIONS: All plumbing installations must be tested and inspected in accordance with the requirements of the Minnesota Plumbing Code. As specified in Minnesota Rules, part 4715.2830,no plumbing work may be covered prior to completing the required tests and inspections. Provisions must be made for applying an air test at the time of the roughing-in inspection as outlined in Minnesota Rules, part 4715.2820, subpart 2, of the code. A manometer test, as specified in Minnesota Rules, part 4715.2820, subpart 3,is required at the time of the finished plumbing inspection. It is the responsibility of the contractor/installer to notify the Minnesota Department of Labor and Industry when an installation for a state contract job, licensed facility, or project in an area where there is no local administrative authority is ready for an inspection and test. To schedule inspections, contact the state plumbing standazds representative for your region,or call Jim Peterson at 651/284-5889. REQiTIItEMENT(S): I. The new sink shall not connect to the vertical riser serving the three-comparhnent sink as is currently shown. Since a tell-tale floor drain is required on the three-compartment sink's dedicated fixture branch, the floor drain must specifically serve that sink prior to connecting to any other fixtures (see Minnesota Rules, part 4715.1390, subpart 2). 2. A three-compartment sink, a hand sink, and a floor drain total7 drainage fixture units and are shown discharging through a 2 inch pipe. Since a 2 inch horizontal drain can only accommodate 6 fixture units at a slope of 1/4-inch per foot, this portion of the system is undersized(see Minnesota Rules, part 4715.2310, subpart 2). 3. It is recommended that a cleanout be provided where new waste and vent piping connects with existing plumbing to facilitate required testing of the new installation. 4. Verify that the existing water supply and waste systems are sized to accommodate the added fixtures(see Minnesota Rules, part 4715.3800 and part 4715.2310). Aladdin Plumbing Plan No. 100929 Page 2 December 30,2009 5. Materials used for water distribution piping must comply with the standaxds specified in Minnesota Rules, part 4715.0520. Copper water distribution piping must meet ASTM Standard B88. 6. Materials used for drain, waste, and vent systems must comply with the standards specified in Minnesota Rules, part 4715.0570 through part 4715.0600. 7. The water distribution system shall be disinfected in accordance with Minnesota Rules, part 4715.2250. 8. The plumbing system shall be tested in accordance with Minnesota Rules, part 4715.2820. NOTE(S): 1. The scope of this project consists of remodeling an existing building. Installation includes a kitchen sink and all associated waste, vent, and water supply piping. 2. The building is served by existing municipal sewer and water services. 3. The plans and specifications were prepared by a licensed plumber. Only the plumber who has prepared the plans may use the plans for construction. If another plumber is contracted to install the plumbing, they must submit their own plans and specifications for the proj ect. 4. This plan review is for the plumbing systems only and does not pertain to the licensing requirements for the facility. The licensing authority will report separately on any licensing requirements which must be met. Additional plans, information and fee may be required by the licensing authority for their review. Please note that changes to the plumbing system may be required as a result of their review. Revised plumbing plans must be submitted showing any significant changes to the plumbing system. Authorization for construction in accordance with the approved plans may be withdrawn if construction is not undertaken within a period of two yeazs. The fact that the plans have been approved does not necessarily mean that recommendations or requirements for change will not be made at some later time when changed conditions, additional information, or advanced lrnowledge make improvements necessary. When the body's circulation and elimination systems become impaired, toxins and waste materials become trapped in the vulnerable fat cells of the connective tissue. This wrap uses circulation and gentle pressure to flush toxins and waste materials from the system through internal detoxification of the body. Stored toxins are permanently removed at the cellular level through the circulatory and lymphatic systems. This creates a permanent inch loss of up to 14 inches, while improving general health, nourishing the skin, and contouring the soft areas of the body. *After my first wrap, I had a 15 inch loss! -JB The Wrap Process M'LIS Body Contour Wraps are only provided by a trained spa professionals. The process is sanitary, warm, and comfortable. The wraps act upon all factors that cause body misshaping, including excess fluids, trapped waste, cellulite, and tissue weakness. With this ability, along with the natural ingredients in the cream, inch loss is just one of the benefits of the M'LIS Wrap. Other advantages include re-contouring, skin hydration and smoothing, the restoration of blood circulation, and the cleansing of body tissues for improved health. A series of three to six Body Contour Wraps is recommended, with four to seven days between each wrap. If a client is overweight and on a weight loss program, a wrap is suggested for each five to ten pounds lost. • Step One: Consultation. The client fills out a short questionnaire designed to help determine client goals and any pertinent medical information. This may occur the day of the wrap, or in a separate appointment prior to the wrap. A patch test on the inner arm may be done to rule out sensitivity to the cream. If necessary, the wrap may be done using M'LIS Contour Cream for Sensitive Skin. • Step Two: Preparation. The client will come to the wrap appointment freshly bathed, without any lotions or perfumes on the body. The legs should be shaved the night before the wrap, rather than the same day. Client will bring a small bikini swimsuit or bikini panties and bra, along with anklet socks for warmth, which they will change into once inside a private treatment room. http://www.wayzataskincare.com/menu/ 1/26/2011 . Wayzata SkinCare Service menu Page 3 of 5 • Step Three: Skin Exfoliation. The M'LIS Wrap uses a transdermal cream to facilitate inch loss. When the skin is exfoliated with M'LIS BUFF before the cream is applied, dead skin cells are removed, capillaries are dilated to help nutrients get to the cells, and the metabolism of the skin is activated. This increases the inch loss results obtained through the wrap. • Step Four: Measurement. The areas of the body where inch loss will occur are measured and marked to ensure accurate measurement after the wrap. This includes the legs, hips, abdomen, waste, rib cage and upper arms. Dimensions are recorded on a progress chart. • Step Five: Application of Contour Cream. Scientifically formulated body wrap cream is topically applied. It is absorbed into the cell region and stimulates circulation while promoting tissue cleansing. All ingredients are natural and safe.Niacin and niacinamide are both forms of vitamin B-3 that dilate blood vessels. Vitamin A increases metabolic functions. Cassia is an herb that promotes blood and fluid flow. Both cassia and niacin produce sensations of warmth as they are absorbed into the skin. There will be a normal pink tone to the skin as circulation is increased. The cream is not applied to the face or breasts. • Step Six: Wrap. Specialized plastic wrap is used to contour the body and to use pressure points in order to advance the working ability of the cream. Fat storage houses (cell chambers) are reduced, allowing the body's own cleansing process to take over and eliminate excess fluids and waste materials from the body. Plastic wrap helps to hold body heat in, thus aiding the skin in absorption of the cream. The cream is formulated without alums or salts in order to prevent perspiration, ensuring that fluids and valuable minerals are not lost. • Step Seven: Relaxation. Once the client is wrapped, they are reclined and able to relax for one hour, while the cream and wrap are put to work. The client is covered with a blanket to keep warm and maintain circulation at the skin level. Client should not get hot enough to sweat, as this will remove cream from the skin. Unlike a wrap using wet gauze, the client remains warm and comfortable throughout the process. A reling facial or reflexology treatment may be done during this time. • Step Eight: Remove and Re-Measure. The wrap is removed and the client is re-measured in exactly the same places as before. The measuring marks are still visible, so there is no doubt as to the accuracy of the inch loss achieved. "After"measurements are recorded and compared to "Before" measurements. The inch loss results are totaled and recorded on the client progress chart. Client can see results on paper 'n e mirror. • Step Nine: Complete the Treatm t. The excess cream still on the skin is massaged in. Clie t should avoid showering for severa o , produce results. Many clients report an additional inch-loss following the wrap. Because the skin-tightening results are not dependent on a water loss, but on the body's own cleansing function, inch loss won't return as liquids are taken into the body. In fact, the client should drink water immediately following the wrap to help flush out any released toxins or waste. • Step Ten: Continue the Inch Loss Process. The M'LIS Home Care Kit may be used to maintain and increase desired contour and inch loss results in conjunction with the M'LIS Body Contour Program. The kit is especially beneficial for those who have purchased Contour Wraps in a series. • Step Eleven: Enjoy your results!! **Now available! Eminence ORGANIC body wraps!
