Non perntitted events will not be allowed. 5 Sec.13.04 RIGHTS OF SUBJECTS OF DATA , Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself shall be informed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide system;(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying oc refusing ro supply private or co�dential data;and(d)the idenriry of other persons or entides authorized by state or federal law to receive the data. This requirement shall not apply when an individual is asked to supply investigative data, pursuant to section 13.82, subdivision 5, to a law enforcement officer. The commissioner of revenue mav olace the nobce required under this subdivision in the individual income tax or vrooertv tax refund instructions instead of on those forms. Subd. 3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be infornted whether he is the subject of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request, an individual who is the subject of stored private or public data on individuals shall be shown the dara without any charge to him and, if he desires, shall be informed of the content and meaning of that data. After an individual has been shown the private data and informed of its meaning, the data need not be disclosed to him for six months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The responsible authority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority may require the requesting person to pay the actual cosu of making,certifying,and compiling the copies. The responsible authoriry shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays, if immediate compliance is not possible. If he cannot comply with the request within that time, he shall so inform the individual, and may have an additional five days within which to comply with the request, excluding Saturdays,Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private data conceming himself. To exercise this right, an individual shall notify in writing the responsible authoriry describing the nature of the disagreement. 'The responsible authority shall within 30 days either. (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipients of inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The determinadon of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating ro contested cases. November 3, 1999 Tony Patterson 1780 Shadywood Road Orono, MN Re: Second Story Addition 1780 Shadywood Road Orono, MN Commission No. 99248 Gentlemen: The purpose of this letter is to report the findings of a structural engineering review of the foundation system for this residence. ASSIGNMENT J. H. Dahlmeier Engineering Inc. has been retained to provide a structural engineering review of the foundation system to determine if it is structurally � adequate to support a second story addition for the residence located at 1780 Shadywood Road, Orono, MN as directed by Tony Patterson, owner of the property. BACKGROUND This one and one-half story residence is going to be remodeled to a two story structure. The City Building Inspections Department desires an independent structural engineering review of the foundation system to determine that it is structurally adequate to support the new construction. 2434 Commerce Boulevard • Mound, MN 55364 � (612) 472-4746 • FAx (612) 472-4761 4 � Ton�Patterson ' November 3, 1999 � Page 2 of 3 OBSERVATIONS AND COMMENTS 1. A site visit was conducted by J. H. Dahlmeier of J. H. Dahlmeier Engineering Inc. on November 2, 1999 to review existing conditions. 2. The residence is a 30' x 32' single-family structure facing west with full basement constructed of concrete masonry and wood frame superstructure. The exterior masonry foundation walls are load bearing as well as the center wood load bearing wall running north and south all supported on concrete footings. 3. The foundation system consists of the following: a. The foundation system is constructed of concrete masonry 20" thick from slab to 5 feet above floor line and 8" thick for an additional 2 feet for a total of 7 feet. b. This condition is typical except the west half of the north wall, east half of the south wall (stairs) and 6 feet of the west wall (north end) which is constructed of 8" masonry. c. The inside surface of the wall has been parged. d. No significant movement or cracks were observed on the interior. e. On the exterior, small vertical cracks exist at the corners. £ A 6' x 6' storm cellar 7 feet high exists in the northwest corner below the basement floor slab. 4. A structural engineering analysis was completed with the following results: a. The foundation sections with 20" wide foundation wall are structurally adequate to support the new loads and existing earth pressure. b. The 8" thick foundation walls described in paragraph 3b are not structurally adequate. Corrective measures will be required. c. The storm shelter is structurally adequate. 5. No other structural engineering was directed or required including porch foundation system, floor framing and framing over storm shelter. CORRECTIVE MEASURES 6. The 8" masonry walls listed in paragraph 3b need to be grouted full with 2500 psi grout from footing to top of wall. , Tony Patterson � November 3, 1999 Page 3 of 3 PROFESSIONAL OPIIVION 7. It is my professional engineering opinion that based on a visual inspection and structural analysis, the foundation system will be structurally adequate following completion of the corrective measures for construction of the second story. GENERAL 8. The information, observations and opinions stated in this report are based on an inspection made by me. The inspection consisted of a visual walk-through observing exposed elements and thosP accessihle without the removal of finished materials. 9.
