On June 11, 1993 you requested an extension until July 1, 1993 due to personal problems. I have left phone messages for you several times since that date. As of this date, the issue remains unresolved. Therefore, you leave the City no alternative but to initiate legal action. Action will be withheld until December 30, 1993. If you have failed to submit a permit application by that date, citations will be issued and the matter will be refened to the City Attorney for further legal action. Due to the fact that a portion of this application is "after the fact", please submit the required information on that portion as soon as possible.
