The City has begu�the review of building permit application for 2150 Prospect Avenue, received by this
office on June 14, 2 12. Your application is incomplete.
The following items! must be submitted or revised in order for your application to be considered complete
and for the plan revi�ew to continue:
1. Certificate of Survey. Please provide an updated, full size certificate of survey meeting the City's
survey starrdards (enclosed) indicating the location of the proposed deck (and any proposed
grading) as well as all existinq structures, landscaping, retaining walls and hardcover on the
property.
2. Hardcover �Calculations. The subject property is located within the Shoreland Overlay district and
hardcover c�alculations are required. Please have your surveyor complete the enclosed Hardcover
Calculation�IVorksheet reflecting the existing and proposed hardcover levels.
3. Escrow 8� scrow Agreement. Building permits involving grading and/or review by the City's
engineer re uire submittal of an escrow and an escrow agreement. The purpose of the escrow is to
guarantee r imbursement to the City for out-of-pocket costs incurred during the review of your plans.
Additionally'this escrow will guarantee conformance with City Code Chapter 79 relating to erosion
control and�tormwater. If grading is to be conducted, the required escrow amount for this project is
$2,500; if o grading is proposed a $600 escrow will be required. The escrow agreement is
enclosed. The property owner must sign the escrow agreement and submit a check for the
appropriate amount.

To date, the information necessary to complete the review of the application has not been
received. Lylie Oman, Building Official, visited the property and found the project never to
have been stalrted.

State Building Code. This permit is for nly the work described and does
not grant permission for additional or re ated work which requires sepazate
permits. All provisions of laws and ordi ances goveming this type of work

expire and become null and void if cons ction authorized is not
