The commissioner of ro�erme mav place the nodce rewired�nd-r this subdivision in the individual income tax or nropem tax refund instructions inscead of on those forms. Subd.3. Access to data by indi�idual. Upon request to a resgonsible au[horiry,an individual shall be informed whether he u the subject of swred data on individuals,and whe[her it is classified as public,priva�.or confidecrial. Upon his further request,an individual who is the subject of scored private or public dara on individuals shall be shown the dara wi�ou[any c'�rge to him and:if he desires, shall be infocmed of the contenc and meaning of chat data. Afrer an individual has been shown the priva;�daca and in.°ormed of its meaning.rhe dara need not be disclosed to him for six monchs chereaiter unless a dispute or acdon pursuan�w chis secdon is F:nding or addiaonal data on the individual has been collected or creaced. Ihe responsible au�horiry shall provide copies of the private or public data upon requ-x.by the individual subject of[he daca. The responsible authoriry may require rhe requesang penon W pay ehe accual costs of makin¢,cerying,and compiling the copies. The responsible au�hority shall comply immediately,if possible,wi[h any r.quest made punuant to this subdivision,or within five days of the date of the requesc,excluding Saturdays,Sundays and legal holidays,if immediau^.compliance is not possibte. If he cannot comply with the request wichin chac time,he shall so inform�he individual,and may have an addirional five days wiehin which to comply wich che request,exeiuding Saturdays, Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. :�n individual may contest the accuracy or completeness of public or pri�•ate dara conceming himself. To exercise chis right,an individual shall noufy ia wriang�e responsible authoriry describiag che nacure of the disagreemeac. The responsible au�horicy shall wi[hin 30 days either: (a)correct che daa fcund m be inaccurate or incomplete and attempt to noafy past recipiencs of inaccurate or incomplete dara, including recipients named by the individi:�;or(b)�odfy the individual that he believes the data to be correcc.
