� 1 <br /> Builder Acknowledgement Form <br /> Permit #2017-00602 / 3017 North Shore Drive <br /> Builder Representative Name: � �a�i� �c��l-Sor1 <br /> Permit Conditions: Initials <br /> **NOTE CHANGE** Before scheduling an exterior insulation and/or drain tile inspection,a <br /> foundation as-built survey must be submitted and approved by the City or a Stop Work order <br /> will be issued. <br /> Schedule a minimum of one hour for the framing inspection. <br /> Erosion control mechanisms must be installed and inspected by the City prior to any land <br /> disturbing activities. The contractor must provide a minimum of a 24 hour notice prior to <br /> inspection. <br /> Erosion control shall be installed and maintained throughout the entire project and must <br /> remain until vegetation has been established. <br /> No underground sewer within 20 feet of well. <br /> Prior to the issuance of a Certificate of Occupancy an as-built survey and hardcover calculations L1�1 <br /> must be submitted and approved. <br /> In the event of winter or other extended unfavorable weather conditions(which prevent the ! <br /> completion of the exterior improvements and/or as-built survey)a Temporary Certificate of ' <br /> Occupancy(TCO) may be necessary. A TCO requires a$10,000 escrow. <br /> Prior to the release of the escrow funds a final inspection must be completed and all disturbed <br /> areas established with vegetation. Please note established vegetation means perennial cover ` <br /> with a density of 759'0.
