� Sec.13.04 RIGHTS OF SUBJECTS OF DAT?, Subd. 1. Type of data. The righcs of individual on whom che dara is scored or to be scored shall be as set fanh in this secrion. Subd.2. Iaformation reqirired to be given iadividual. An individual asked to supply private or confidenaal data concerning himself shali bt informed of: (a)the purpose and inrended use of the requested data within the collecang rtare agency,political subdivision,or statewide system; (b)whedier he may refuse or is legaliy required to supply the requesud data;(c)any lrnown consequence arising from his supplying or refusiag to supply �„ � privace or confidenrial data;and(d)[he idendry of ocher persoas or enades aurhocized by state or federal law w receive the data. Access to data bp individual. Upon request to a rosponsibte auchoriry,an individual shall be informed whether he is the subject of stored darn on individuals,and whe�er it u ctassified as public,private or co�denaal. Upon his further request,an individual who is the subjecc of scored private or public dara on individuals shatl be shown che data wichouc any charge W him and,-if he desires, shalI be informed of the content and meaning of chat dard. Aher an individuaI has been shown the priva�e dara and informed of iu meaning,che data need not be disclosed co him for six monchs thereaiur unless a dispute or acdon pursuan�to rhis xcaon is pending or addiaonal daca on[he individual has been collected or creaced. The responsible authoriry shall provide copies of the private or public dara upon mqueu by[he individuai subjecc of rhe data. The responsibte authoriry may require[he requesring person to pay the actual cosu of making,certifying,and compiling the copies. The responsibie authoriry shall comply immedia�e[y,if possible,with any reques�made pursuant to chis subdivision,or wichin five days of the dace of the request,exciuding Sacu�days,Su�ays and legal halidays,if immediate compliance is not possible. If he cannot compty wich the requesc within chac time,he shail so inform ehe individuat,and may have an addidonal five days wichin which to comply wich the request,excluding Saairdays, Sundays and legal holidays. Subd.4. Pzocedure when data is not accurate or complete. An individual may contest the accuracy or compte[eness of public or pmate dara concerning himself. To exercise chis right,an individuat shall nodfy in wricng rt►e responsible auehoriry describing the nacure of the disagreemenc. The responsible authoriry shall within 36.days either. (a)correcc the dam faund�be inaccucate or incomplete and atcempt to nodfy past recipienss of inaccurdce or incomplece data, including recipiencs nam�d by che individual;oe(b)noafy the individual thac he believes the data to be coaect Data in dispute shall be disclosed only if the individual's stacemen�of disagreement is inctuded wirh�he disctosed data. The determinarion of the respoasibte authoriry may be appealed punuanc w the provisions of the administrarive procedure act reladng to conrested cases.
