Access to data bv indiridual. Upon requesc to a responsible au�horiry,an individual shall be informed whether he is the subjecc <br /> of stored data on individuals,and whe�her ic is classified as public, private or confidenaal. Upon his further requesc,an individual who is che subjecc <br /> of stor:d private or public data on individuals shall be shown che dara wichout any charge to him and, if he desires, shall be informed of che concznc <br /> and meaning of�hac dara. Afrer an individual has been shown che privaee data and informed of i�s meaning,the data need not be disclosed to him for <br /> six mon�hs[hereaher uniess a dispute or acaon pursuanc to this secdon is pending or addidoaal data on the individual has been collecced or creared. <br /> The responsible authoriry shall provida copies of the privace or public data upon request by the individual subjecc of the data. The responsible authoriry <br /> may require �he requcsring person to pay the actual cosu of makinz, cerrifying,and compiling the copies. <br /> "Che responsible au�horiry shall compiy immediately, if possible, wich any request made pursuant co this subdivision, or within five days of <br /> the dace of rhe request,exciuding Saturdays,Sundays and legal holidays,if immediace compliance is not possible. If he canno�comp(y with the requesc <br /> wichin rhat dme,he shall so inform che individuai,and may have an addidonal five days within which to comply wich the request,excluding Sacurdays, <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An individual may contest[he accuracy or completeness of pubtic or private <br /> data cor.cerning himself. To exercise this right,an individual shall noafy in wridng[he responsible auchoriry describing[ho nacure of the d'uagreement. <br /> The responsible au�horiry shal(within 30 days either: (a)correct the data found to be inaccurate or incomplece and attempc to noafy past recipients of <br /> inaccurace or incomplete data, inciuding recipien�s named by che individual; or(b)nodfy the individual tha�ha believes rhe daca to be correcc. Data <br /> in dispu�e shall be disclosed only if the individual's stacemenc of disagreement is included with the disclosed data. <br /> The decerminaaon of the responsible au�horiry may be appealed pursuant to the provisioos of the adaunistrarive procedure act relaring to <br /> contested cases. <br /> DATA PRiVACY ADVISORY <br /> In accordance wich M.S. 13.04, Subd.2, "Rights of subjects of data", we would like to inform you that your request <br /> for a pernut or license from the Ciry of Orono or any of its departments may require you to furnish certain private or <br /> confidential information. <br /> You are notified that: <br /> 1. The information you furnish will be used to determine your qualification for the permit or license requested. <br /> 2. You may refuse to supply data, buc refusal may require that the City deny the permit or license. <br /> 3. If your requested permit or license requires Council action to approve, some information may become <br /> public. <br /> 5.
