Staff conducted a preliminary review based on the information provided and recommends the following <br /> items be submitted or revised in order for your appiication to be considered complete and for the plan review to <br /> continue: <br /> 1. Zoning Disclosure Form. Attached is a Zoning pisclosure Form. Please have the owner,Barbara Burweii sign <br /> the form and return. <br /> 2. Certifrcate of Survey. 7he 2012 survey submitted with the application shows a shed/accessory structure on <br /> the westerly side of the property. f believe the shed has been removed. Please advise if the cement slab will <br /> be demolished or if it will remain. <br /> 3. Estrow Agreement. <br /> a. Based on our records all previous escrows submitted by the Burwell's have been refunded around <br /> 09-f2-2011;09-09-2013 and 05-23-2016. <br /> b. Permits involving grading and/or review by the City's engineer require submittaf of an escrow and an <br /> escrow agreement. The purpose of the escrow is to guarantee reimbursement to the City for out-of- <br /> pocket costs incurred during the review of your plans. Aciditionally this escrow will guarantee <br /> conformance with City Code Chapter 79 relating to erosion control and stormwater. The required <br /> escrow amount for this projed is$2,500. A blank,signed escrow agreement was submitted wlth the <br /> application. Some of the missing information has been completed, a copy is attached. Please submit <br /> a che�k for$2,500 and compfete Item Number 8 of the escrow agreement stating who the escrow <br /> money should be refunded to. <br /> 4. Well Abandonment. Provide us with a copy of the well abandonment paperwork. <br /> 5. Minnehaha Creek Watershed Distrlct {MCWD). Your project may trigger the Minnehaha Creek Watershed <br /> District's (MCWDs) permitting requirements; please contact the MCWD directly at 952-471-0590 regarding <br /> your project. Please note,the City of Orono will not issue a building permit without a copy ofthe MCWD permit <br /> or documentation stating the proposed project dces not trigger any of their perrnitting requirements. <br />
