The righcs of iadividual on whom the data is scored or to be swred shall be as sec forth in this secaon. <br /> Subd.2. Information reqirired to be givm individual. An individual asked w supply privare or confrdendal data concerning himself shall <br /> be informed of: (a)the purpose aad inunded use of the requesred data within the collecang ttate agency,polirical subdivision,or statewide system; <br /> (b)whecher he may iefuse oi is legally required to suppty the requesud data;(c)any lmown coasequence arisiag hom his supplying or refusing w supply <br /> private or conSdendal data;and(d)the idendry of other persans or endaes authorized by stace or federsl law co�eceive the dara. This requirement"sha(1 <br /> not apply when an individual is asked'w supply invesagadve dara,punvant to secaon 13.82,subdivision 5,to a law eaforcemenc officer. <br /> The commissioner of revenue mav olace the noace rewired under this subdivision in the individual income tax or oropectv tax refund <br /> instcucdons instead of on those forms. <br /> Subd.3. Accas to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject ' <br /> of stoced data on individuals,aad whaher it is class�ed as public,priva[e or confidenaal. Upon his further request,an individuai who is the subject <br /> of stored private or public dara oa iadividuals shall be shown the data wirhouc any charge to him and:if he desires,shall be infotmed of the content <br /> and meaaing of that data. Afur an iwdividual has been shown the private dara and informed of ia meaning,the dara need not be disclosed to him for <br /> six mondu thereafter unless a dispute or acdon pursuanc w rhis secaon is pending or addiaonal dara on the individual has been collecud or creared. <br /> The responsible authoriry shall provide copies of the private or public data upon mquest by the individual subject of the data. The responsible authority <br /> may require the requesdng person to pay the acaial costs of making,cerafying,and compiling the copies. <br /> The responsible authoriry shall comply immediatety,if possible,with any tequest made punuant w this subdivision,or wichin five days of <br /> the date of the request,exctuding Saardays,Sundays and legal holidays,if immediate compliaace is noc possibie. If he caanot comply wi�h the request <br /> within that time,he shall so inform che individual,and may have an addidooai five days within which to comply wirh the request,excluding Sanudays, <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An individual may contest the accurscy or completeness of public or private <br /> data concerning himself. To exercise chis right,an individual shall nodfy in wridag the respoasible aurhoriry describiag the nature of the disagreemenc <br /> The responsible authoriry shall within 30 days eichee (a)correct the data found ro be inaccurate or incomplete and aaempt to nodfy past recipients of <br /> inaccurau or incomplete da[a, inciuding tecipients named by the i�ividual;or(b)noafy the individual thac he believes the data to be corcect Data <br /> in dispute shall be disclosed only if the individual's statemenc of disagreement is included with ehe disclosed data. <br /> The determinaaon of the responsibie authoriry may be appealed pursuant to the provisions of the administradve procedure act reladng to <br /> contesced cases. <br /> DATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04,Subd.2, "Rights of subjects of data",we would like to inform you that your request <br /> for a permit or license from the City of Orono or any of its departments may require you to furnish certain private or <br /> confidential information. <br /> You aze notified that: <br /> 1.
