� <br /> Sec.13.04 RIGFITS OF SUBJECTS OF DATA <br /> Subd. 1. Type of data. The rightt of individual on whom the daa is stored or to be stoced shall be as set focth in this secdon. <br /> Subd.2. Information req�ared to be given individual. An individual asked W supply private or confidential data concecning himself shall <br /> be informed of: (a)the purpose and intended use of the requesoed data wid�in the collecting 3�tate agency.political�bdivision,or statewide system; <br /> (b)whether he may nfuse oY is legally required to supply[he requesud data;{c)any lmown consequence arising from his suPP�Y�B or refusing W supply <br /> privau or wnfidential data;and(d)the identiry of othec persons or entities authorized by stau or federal law to'receive du dala. Access to data by individual. Upon rcquest to a responsible authoriry,an individual shall be informed w6ether he is the subject <br /> of stored data on individuals,and whether it is ctassified as public,private or confiden6al. Upon his further request,an i�ividual who is the subject <br /> of stored private or public data on individuals shall be shown the data without any c6arge co him and;if he desires,shall be infocmed of the contenf <br /> and meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for <br /> six months theroafur unless a dispute or acaon pursuant to this secrion is pending or addiaonal data on the iadividual has been collecud or created. <br /> The responsibie authority shall provide copies of the private or pub(ic data upon request by the individual subject of the data. The respoasible suthority <br /> may req�ire the requesting person to pay the actual costs of making,certifying.and compiling the copies. <br /> The responsible authoriry shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of <br /> the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request <br /> within that time,he shall so inform the individual,and may have an addiaonal five days within which W comply with the request,excludicig Saturdays, <br /> Sundays and legat holidays. <br /> Subd.4. Prceedure when data is not accurate or complete. M individual may contest the accuracy or completeness of public or private <br /> data conceming himself. To exercise this right,an individuai shall nodfy in writing the responsible authority describing dte nature of the disagreement <br /> The responsible authoriry shall within 30 days either. (a)correct the data found m be inaccurate or incomplete and attempt to nodfy past recipients of <br /> inaccurate or incomplete daa, including recipients named by the individual;or(b)notify the individual that he believes the data to be correcG Data <br /> in dispute shall be disclosed only if the individual's statement of disagreement is included with the disciosed data. <br /> The determination of the responsible authoriry may be appealed pursuant w the provisions of the adminis�adve procedure act relating w <br /> contested cases. <br /> DATA PRNACY ADVISORY <br /> In accordance with M.S. 13.04,Subd.2, "Rights of subjects of data", we would like to inform you that your request <br /> for a permit or license from the City of Orono or any of its departments may require you to fiunish certain private or <br /> confidential infortnation. <br /> ' You are notified that: <br /> 1.
