Information.re�quired to be given in�ivi�ual- An.individual esked to <br /> � ' supnly private or confidential c3ata concerning himself shall be informed of: (�) the <br /> purpose and intended tise �f the requested deta within the collecting state agency, <br /> political subdivision, or statewide s}�stern; (b) whether he may refuse or is legally <br /> required to supply the requested date; (c) any known consequence arising from his <br /> supplying or refusing to supply private or confidential data; end (d) the identity of <br /> other persons or entities authorized by stste or federel law to receive the data. This_ <br /> requirement shell not apply when an individuel is asked to supply investigative datE, <br /> pursuant to section 13.82, subdivision �, to a law enforcement officer. <br /> The commissioner of revenue mav place the notic� reauired under this <br /> subdivision in the individual income tas or nro�ertv tex re�und instructions instead oi <br /> on those orms. <br /> Subd. 3. � Aecess to �ata by in�vi�uaL IIpon request to a res�onsible � <br /> euthority, an individual shail be informed whether he is the subject of stored data on <br /> individuals; and whether it is clessified �s public, private or confidential. IIpon his <br /> f�rther request, an individual who is the subject of stored private or pul�lic date on <br /> indivi�uels shall be shown the data without $ny charge to him and, if he desires, shell <br /> 6e informed of the content 8nd meariing of that data. After an individual has been <br /> shown the private data 8nd informed of its mesning, the data need not be �sclosed io <br /> him for six months thereafter unless a dispute or ection pursuant to this s�ction is <br /> , � pending or additional data on the individuel hes been collected or created. The . <br /> responsible authority shall provide capies of the private or public data unon request hy <br /> the individuPl subject of the data. The responsible euthority may reauire the <br /> reouesting person to pay the acruel costs of maldng, c�rtifying, and compiling �he <br /> CODIPS. <br /> The responsibla authority shall comply immediately, if possible, with eny request <br /> made pursuant to this subdivision, or within five days of the date of the requst, <br /> excluding Saturdst�s, Sundays and legal holideYs, if �immediate complignes is not <br /> possible. If he cannot comply with the reouest within that time, he sheIl so inform the . <br /> individusl, and may have 8n addition�l five days wiihin which to comply with the <br /> request, excluding Saturds��s, Sundays end Iegal holidsys. -- - <br /> Subd. 4. Proce�se when �ata is not accss$te or complete. An individual mEy <br /> contest the accuracy or completeness�of public or privste data concerning himself. To <br /> exercise this right, an individuat sh�Il notify in writing the responsible authority � <br /> describing the nature of the disagreemen� The resporLsible euthoritq shall within 30 <br /> days either.
