What's the max funding amount for/�ctive Places demonstration prajecfs? <br /> A. No organization will be awarded more than $20,000. <br /> Q. How much money total is available for these prajects? <br /> A.Approximately$160,000 to$180,000. <br /> Q. When da the �rojects have to be completed by? <br /> A. While there is no specific date for when your projects must take place,funding must be used by <br /> December 31, 2015. <br /> Q. tNhat are the euaEuations requirements for these project�? <br /> A. Each project is required to submit a report to Blue Cross after completion of the project. For projects <br /> that extend beyond 2015, reports are due by the end of the calendar year.Additional evaluation <br /> requirements will be project dependent. <br /> Q. What are the eligibility requirements for organizations to apply? Do organizations have to <br /> be under Blue Cross Insuranre? <br /> A.The eligibility requirements are on pages 5 and 6 of the RFP.This funding is open to all organizations <br /> regardless of their insurance carriers. <br /> Q. What advice da you have for organizations who don't yet have firm p{ans for their <br /> projects? <br /> A. It is not always possible to have all of the information you need when planning a new initiative. For <br /> the purposes of this funding opportunity,at minimum you should know the community you want to <br /> target and your plan to engage community members.We advise having the type, duration,and <br /> frequency of your demonstration project in mind; however,you may find in the community engagement <br /> process that your idea will require revisions. This is expected and encouraged. Keep in mind, however, <br /> that your budget template will be scored on its appropriateness, meaning the more you know up front, <br /> the better you will be at getting solid budget numbers. If you are unsure,start by considering the types <br /> of materials, permits, labor,etc.,you might need and the cost associated with each so that you can <br /> justify your budget. <br /> 1 <br />
