Injormatian required to be given individuaG An indivrdua!asked to supply private or confrdentia!data concerning himselj shall be informed of. (a)the purpose and intended:�se ofthe requested data within the collectingstate agency,politicalsubdivision,or statetivide system;(bJ whether he may refuse or rs legally required to supply the requested data;(c)any knotivn consequence arising from his supplying or refusing to supply private or confidentia!data;and(d)the identity of other persons or entities au[horrzed by state or federal law to receive the dala. This requirement shall not apply when an individual is asked to supply investrgalive data,pursuant to section 13.82, subdivision S, to a Imv enforcement o�cer. The commrssioner of revenue may place the notice required under this subdivrsron rn the individual income tax or property 1ax refund instructions rnstead of on those forms. Subd.3. Access to data by individual. Upon request to a responsrble authority,an rndivid:�al shall be informed whether he is the subject of stored data on individuals, and whether it rs classified as public,private or confrdential. Upon hrs further request,an indlvidua!who is the subject of sfored private or public data on rndividuafs shal!be shown the data without any charge to him and, if he desires,shall be informed oJ the content and meaning oJthat data. Afier an indivrdua!has been shotivn the private data and informed of its meaning, the data need not be disclosed to him for srx months thereafter unless a dispute or actron pursuant to this section is pending or additronal data on the rndividttal has been collected or created. The responsrble authority shal!provide copies ojthe prrvate or public data:rpon request by the individual subject of the data. The responsible authoriry may require the requesting person to pay the actual costs of making,cert�ing,and compiling the copies. The responsible authority shall comply immediately,rf possible,with any reqt�est made pursuant to thrs subdivision,or within ftve days ojthe date oJthe request,excluding Saturdays,Sundays and lega!holidays,if immediate compliance is not possi6le. Ifhe cannot comply with the request within that time,he shall so inform the indivrdual,and may have an additional five days within tivhich to comply with the req:�est,excluding Saturdays,Sundays and lega!holidays. Subd.4. Procedure when data is not accurate or complete.An individual may contest the accuracy or completeness ofpublic or private data concernrng himself. To exercise this righi, an individual shal!notrfy in writing the responsrble authoriry describing the nature ojthe disagreement. The responsrble aulhoriry shal!ivithin 30 days either: (a)correci ihe data found to be inaccurate or incomplete and altempt to notify past recrpients of inaccurate or incomplete data,including recipients named by the individual;or(b)not�the individual that he believes the data to be correct. Data in drspute shall be disclosed on/y rf the rndivrdual's statement oJdrsagreement is included wrth the disclosed data. The determination of the responsible authoriry may 6e appealed pursuant ta the provisions of the admrnistrative procedure act re[ating to cantested cases.
