i t Sec.13.04 RIGHTS OF SLJB.TECTS OF D�Ta Subd. 1. Type of data. The righcs of individual on whom che data is stored or to be stored shall be as set forth in this secdon. Subd.2. Information required to be�ven indiridual. �,n individual asked to supply private or confidenaal data concerning himself shall be informed of: (a) che purpose and inceoded use of the requesred dara within the collecang'srace agency, poliacal subdivisio�, or sca�ewide system; (b)whether he may refuse or is legally required to supply the requestPd data:(c)any 4mown consequence arising from his supplying or refusing co supply priva�e or confidenaal data;and(d)che idenary of o�her persons or enaaes au�horized by stacz or federal law to receive the data. This requirement shall no�apply when an individual is asked �o supply invesagaave da�a, pursuanc to seccion 13.82, subdivision 5, co a law enforcemeat o�cer. The commissioner of revenue mav place the notice required under �his subdivision in the individual income tax or prooer[v taz refund instrucaons instead of on those forms. Subd. 3. ?,ccess to data by individual. Upon requesc[o a responsible auchoriry,an individual shall be informed whether he is the subject of stored data on individuals, and whe�er it is classified as public, priva[e or confidenoal. Upon his furrher request,an individual who is the subjecc of scored private or public data on individuals shall be shown the data wi�hout any charge to him and, if he desires, shall bz informed of che concent and meaning of that data. After an individual has been shown che private data and informed of its meaning, the data need no[be disclosed to him for six months [hereafter unless a dispure or acdon pursuanc to this secaon is pending or addirional data on rhe individual has been collecced or created. The responsible au[horiry shall provide copies of the private or public da[a upon request by the individual subject of the data. The responsible authority may �equire the requesang person to pay the accual coscs of making, cerafying, and compiling the copies. The responsible authoriry shall comply immedia[ely, if possible, with any request made pursuant to this subdivision, or within five days of the date of the request,excluding Sanirdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request within that rime,he shall so inform[he individual,and may have an addirional five days within which to comply wi[h rhe request,excluding Sacurdays, Sundays and legai holidays. Subd.4. Procedure whea data is not accurate or complete. An individual may co�cesc the accuracy or completeness of public or private data conceming himself. To exercise chis righc,an individual shall norify in wridng[he responsible authoriry describing the nature of the disagreemen[. The responsible auehoriry shafl within 30 days either: (a)correct the data found to be inaccurace or incomplete and attempt to noafy past recipients of inaccurate or incomplece da[a, including recipients named by [he individual; or(b) noafy [he individual that he believes the data to be correct. Data in dispute shali be disclosed only if the individual's statement of disagreement is included wi�h the disclosed data. Th�decerminacion of the responsible auchoriry may be appealed pursuanc co the provisions of the adaunistradve procedure act relating to contested cases. DATA PRIVACY ADVISORY In accordance with M.S. 13.04, Subd. 2, "Rights of subjects of data", we would like to inform you that your request for a permit or license from the City of Orono or any of its departments may require you to furnish certain private or confidential information. You are notified tha[: 1. 1fie information you furnish will be used to determine your qualification for the permit or license requested. 2. You may refuse to supply data, but refusal may require that the City deny the permit or license. 3. The information may be shared with other local, state or federal a�encies to the extent necessary to process the permic or license. 4.
