The righcs of individual on whom the data is stored or co be stored shall be as set forth in this secdon. <br /> Subd.2. Information reqirired to be given individual. .�.n individual asked to supply private or confidendal dara concerning himself shall <br /> be informed of: (a) [he purpose and inrended use of the requesred data within the collecting"stat�agency,polidcal subdivision,or sratewide sysum; <br /> (b)whether he may refuse oY is legally required to supply[he requested data;(c)any lmown coasequence arising fmm his supplying or refusing to supply <br /> privace or confidenria!data;and(d)the ideanry of ocher persons or enddes authorized by state or federal iaw to receive the data. 'Ihis requiremenc shall <br /> not apply when an individual is asked to supply invesrigadve daca,punuant to secdon 13.82, subdivision 5, to a law enforcement o�cer. <br /> The commissioner of revenue mav piace the nodce rewired under this subdivision in the individual income tax or propem tax refund <br /> instructions inscead of on those forms. <br /> Subd. 3. Access to data b�individual. Upon request to a responsible authoriry,an individual shall be informed whecher he is the subject <br /> of stored data on individuals,and wherher it is class�ed as public, priva[e or confidenaal. Upon ivs further request, an individual who is the subjec� <br /> of stored private or public data on individuals shall be shown[he data wichout any charge to him and, if he desires, shall be informed of[he conter.t <br /> and meaning of rhat data. Aher an individual has been shown che private data and informed of irs meaning,the dara need not be disclosed to him for <br /> six months thereafter unless a dispute or acdon pursuan�to chis secrion is oending or addidonal data on the individual has been collected or crea�ed. <br /> The responsible authoriry shall provide copies of the privace or public data upon request by the individual subject of che dara. The responsible authoriry <br /> may require the requesring person to pay the acrual cosu of makin¢, cemfying, and compiling the copies. <br /> The responsible authoriry shall comply immediacely, if possible, with any request made pursuant to this subdivision,or within five days of <br /> the date of the request,excluding Saturdays,Sundays and legal holidays,if immediace compliance is not possible. If he cannoc comply with the requesc <br /> within�hat time,he shall so inform the individual,and may have an addidonal five days within which to comply with the request,excluding Saturdays, <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An individua!may contest the accuracy or completeness of public or pri�•ate <br /> data concerning himself. To exetcise chis right,an individual shall noufy in wriang the responsible authoricy describing�he nacure of the disagreement. <br /> The responsible authoriry shall within 30 days eicher: (a)correc�che data found to be inaccurate or incomplete and attempc to nodfy pasc recipients of <br /> inaccurate or incomplete data, including recipients named by ehe individual; or(b)nodfy the individual tha[he believes �he data to be correcc. Data <br /> in dispuce shall be disclosed only if the individual's statemenc of disagreement is included with the disclosed data. <br /> The decerminaaon of ehe responsibte authoriry may be appealed pursuanc to the provisions of the adminis�dve procedure act reladng to <br /> contested cases. <br /> DAT� PRIVACY ADVISORY <br /> In accordance with M.S. 13.04, Subd.2, "RiQhts of subjects of data", we would like to inform you that your reauest <br /> for a permit or license from the Ciry of Orono or any of its departments may require you to fumish certain private or <br /> confidential information. <br /> You are notified that: <br /> 1.
