� '� <br /> Sec.13.04 RIGH'I'S OF SUB.TECTS OF D?�T�1 <br /> Subd. 1. Type of data. 'Ihe rights of individual on whom[he data is stored or[o be scored shall be as set for[h in this secrion. <br /> Subd.'_'. Information reqirired to be�vea individual. An individual asked to supply private or confidendal dara conceming himself shall <br /> be informed of: (a)the purpose and intended use of the requested data within the collecang"state agency, polidcal subdivision,or sratewide system; <br /> (b)whether he may refuse oi is legally required to supply[he requested data;(c)any lmown coasequence arising from his supplying or refusing to supply <br /> privace or confidenrial data;and(d)the idenriry of other persoas or enddes authorized by state or federal law to receive the data. This requirement shall <br /> not apply when an individual is asked to supply invesd¢adve dara,pursuant to secdon 13.82, subdivision 5, to a law enforcement o�cer. <br /> The commissioner of revenue mav place [he nodce reauired under this subdivision in the individual income tax or propem rax refund <br /> instructions instead of on chose forms. <br /> Subd.3. Access to data bp individuai. Upon reques[to a responsibfe authoriry,an individual shall be informed whether he is the subject <br /> of stored data on individuals, and whe[her it is classified az public, privace or confidendal. Upon his further request,an individual who is[he subjecc <br /> of stored private or pubtic data on individuals shall be shown the dara wichout any charge to him and,�if he desires, shall be informed of the content <br /> and meaning of ehat data. Aftzr an individual has been shown the private data and informed of i[s meaning, the data need not be disclosed to him for <br /> six months thereafter unless a dispute or action pursuanc co ehis secrion is oending or addidonal data on the individual has been coilected or crea�d. <br /> The responsible authoriry shall provide copies of the privace or public data upon reques[by the individua!subject of the data. The responsible authority <br /> may require the requesdng person to pay the actual cosrs of makin¢,cerufying,and compiling the copies. <br /> The responsible authoriry shall comply immedia[ety, if possible, with aay request made pursuant to this subdivision,or within five days of <br /> the date of the requesc,ezcluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot compiy with the request <br /> within tha[time,he shall so inform the individual,and may have an addidonal five days within which to comply wi[h the cequest,excluding Sarurdays, <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or pri�•ate <br /> data concerning himself. To exercise this right,an individual shall nodfy in wriring the responsible authoriry describing rhe nature of the disagreement. <br /> The responsible auchorirv shall within 30 days eicher: (a)correct the data found to be inaccurate or incomplece and attempt to nodfy past recipienn of <br /> inaccurate or incomplece data, including recipien�s named by ehe individual; oc(b)nodfy the individual tha[he believes �he data to be correct. Data <br /> in dispute shall be disciosed only if the individual's statemenc of disagteement is included with the disc(osed data. <br /> The de[erminadon of the responsible authoriry may be appealed pursuant co the provisions of[he administr2dve procedure act reladng to <br /> contested cases. <br /> DATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04, Subd. 2, "Ri�ts of subjects of data", we would like to inform you that your request <br /> for a pernut or license from the City of Orono or any of its depar[ments may require you to fumish certain private or <br /> confidential information. <br /> You are notified that: <br /> 1. The information you furnish will be used to determine your qualification for the permit or license requested. <br /> 2. You may refuse to supply data, but refusal may require that [he City deny the permit or license. <br /> 3. The information may be shared with o�her local, state or federal a�encies to the extent necessary to process <br /> the permit or license. <br /> 4. If your requested permit or license requires Council action to approve, some information may become <br /> public. <br /> j.
