. � <br /> Sec.13.04 RIGHTS OF SUBJECTS OF DaTa <br /> Subd. 1. Type of data. The riehrs of individual on whom tha data is sto:�or to be scored shall be as set forth in this secdon. <br /> Subd.2. Information reqirired to be given individual. r1n icdividuai askc3 to supply privace or confidendal data concerning himself shall <br /> be informed of: (a)the purpose and in�cnded use of the requesccd data v.:�in the collccring"state agency,poliacal subdivision,or sratewide system; <br /> (b)whecher he may refuse or is legally required co supply che requesied dan;(c)any�own coasequence arisin3 from his supplying or refusing to supply <br /> priva�e or conndenaal data;and(d)the idenacy of orher persons or enriries aachorized by state or federal law to receive the data..This requiremen�shall <br /> noc apply when an individual is asked to supply invesaeadve dac;,pursu�:c to seccoa 13.82,subdivision 5, to a law enforcemenc o�cer. <br /> The commissioner of r�venue r�av place the noace reauired erd�r this subdivision in the individual income tax or oropem tax refund <br /> instructions ins�ead of on those forms. <br /> Subd. 3. Access to data by indi�-idual. Upon requesc to a r.sponsible ae[horiry,an individual shall be informed whether he is the subject <br /> of s�ored data on individuals, and whecher it is classified as public, priva4 or conftd=r.aa1. Upon his further request, an individual who is the subjecc <br /> of scored private or public data on individuals shall be stiown the dara wiu.out any c:.arge to him and,�if he dzsires, sball be informed of the contenc <br /> and meaning of[hat data. Afrzr an individual has been shown the priva��ae�and i�:onned of its meaning,the dara need not be disclosed to hicn for <br /> six mon�hs chereaiter unless a dispute or acaon pursuan�to [his secrion is�ending o�addiaonal data on the individual has been collecced or crea�ed. <br /> The responsible aurhoriry shall provide copies of the privace or public data c�on reques:by the individual subject of the data. The responsible authoriry <br /> may requir� the requesring person to pay the actual coses of makin¢,ce.�:;ing, and compiling the copies. <br /> The responsib(e aurhoriry shall comply immediately, if possio?_,wich an}'r:quest made pursuant to chis subdivision,or wichin five days of <br /> the date of che request,excluding Saturdays,Sundays and legal holidays,ii iremediace compliance is not possible. If he cannot comply with the requesc <br /> wichin tha�dme,he shall so inform che individual,and may have an addicer.al Five days wichin which to compiy wi�h the request,excluding Saturdays, <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An indivi�uzl may contest tht accuracy or completeness of public or pri��ate <br /> data concerning himself. To exercise this rieht,an individual shall norify ia wridng c:.:responsible authoriry describing che nacure of the disagreemeac. <br /> The responsible authoriry shall within 30 days either: (a)correcc the dac3 icund to be inaccurate or incompiete and attempt to norify pasc recipiencs of <br /> inaccura�e or incomplete data, inciuding recipients named by[he individL�i;or(b) r.oafy the individual that he beliaves the data to be correct.
