Access to data by indiridual. Upon reques�to a responsible authority,an individual shall be informed whather he is ehe subjecc <br /> of sto�ed daca on individuals,and whecher it is classified as public,privace or confidendal. Upon his furcher requesc,an individual who is[he subje:� <br /> of stoced private or public data on individuals shall be shown�he data wichout any charge to him and;if he desires, shall be informed of the contenc <br /> ar.d meaning of that data. After an individual has been shown the priva[e dan and informed of i�t meaning, the dara ne:d aot be disclosed to him for <br /> six monats[hereafter unless a d'upute or acaon pursuant to this secdon is pending or addidoaal dara on the individual has been coilected or crea[ed. <br /> 'Ihe responsible autl:oriry shall providz copies of the privare or public data upon request by the individual subject of the data. The responsible authoriry <br /> may require [he requesdng person to pay the accual costs of making, cerdfying, and compiling[he copies. <br /> 'Ihe responsible authoriry shall comply immediatzty, if possible, wich any cequest made pursuant to this subdivision,or wichin five days of <br /> the date of che reques�,excluding Sacurdays,Sundays and legal holidays,if immedia[e compliance is not possible. If he cannot comply with the request <br /> within that dme,he shall so inform the individuai,and may have an addidoaal five days within which to comply wich the request,ezcluding Saturdays, <br /> Sundays and legal holidays. <br /> Subd.4. Procedure when data is not accurate or complete. An individual may contest the accurdcy or completeness of public or private <br /> dara conceming himself. To ezercise�his right,an individual shall noaEy in wriring d'ie respotuible au[horiry describyng the nature of the disagreemenc. <br /> The responsible au�horiry shall within 30 days ei�:er: (a)correcc rha data found to be inaccurate or incomplete and aaempc to nodfy past recipienrs of <br /> inaccurate or incomple�e data, including �ecipien�s named by cbe individual; or(b)nodfy the individual that he believes [he dan to be correcc. Data <br /> in dispute shall be disclosed only if the individual's statemenc of disagreement is included with the disclosed data. <br /> The determinacion of the responsible auchoriry may be appealed pursuanc to the provisions of tha admuusaarive procedure act relacing to <br /> contesced cases. <br /> DATA PRIVACY ADVISORY <br /> In accordance with M.S. 13.04, Subd.2, "Ri�hts of subjects of data", we would like to inform you that your request <br /> for a pernut or license from the City of Orono or any of its departments may require you to furnish certain private or <br /> confidential information. <br /> You are notified that: <br /> 1. The i��ormation you furnish will be used to decemu�e your qualification for the permit or lieense requested. <br /> 2. You may refuse to supply data, but refusal may require that the City deny the perm.it or license. <br /> 3. 'Ibe information may be shared wich other local, state or federal agencies to the e�tent necessary to process <br /> the pernut or license. <br /> 4. If your requested permit or license requires Council action to approve, some information may become <br /> public. <br /> 5.
