< < <br /> Sec.13A4 RIGHTS OF SUBJECTS OF DATA <br /> Subd. 1. Type of da[a. The iights of individual on�vhom the data is stored or to be stored shall be as set forth in this section. <br /> Subd.2. Infotmation required to be given individuaL An individual asked to suyply pnva[e or confidentiul data conceming hi�uself shall be <br /> informed of: (a)dte puipose and intended use of dte requested da[a within the collecting state agency,political subdivision,or statewide system;(b) <br /> whe�her he may refuse or is legally required to supply�he requested da[a;(c)any knowu consequence arising from his supplying or refusing to supply <br /> private or cor,fidential data;and(d)the identity of other persons or entities attthorized by state or federal law to receive the data.This requirement shall <br /> not apply wllen an individual is asked[o supply investigative data,pursuant ro section 13.32,subdivision 5,to a law enforcement officer. <br /> The commissiotter of revenue ma�place the notice required under this subdivision in the individual income tax or pro�er[v tax refiind <br /> inse�uctians instead of on those fomis. <br /> Subd.3. :�ccess to data by individual. Upon request to a respoiuible authority,an individual shall be inFornied whether he is the subject of <br /> stored data on individuals,and«�hether it is dassif;ed as public,priva[e or confidential. Upon his fi�rther request,an individual who is the subject of <br /> stored privatc or public data on individuals shall be shown the data�vithout any charge co him and,if he desires,sltall be infomted of ttte content and <br /> meaning of that data. After an individual has been sho«�n the private data and int'ormed of its meaning,the data need not be disclosed to him for sis <br /> months thereafter unless a dispute or action ptu�suant to this section is pending or additional data on the individuul has been collecred or created. The <br /> responsible audtority shall provide copies of the private or public data upon request by dte individual subject of the data. The responsible authoricy <br /> may require dte requestin�person to pay die ac:ual costs of mukin¢,certifying,and compiling che copies. <br /> The responsible authority shall comply immediately,if possible,with any request made pursutint to dlis st�bdivision,or wichin five days of <br /> the date of the requcst,e�cludin�Saturdays,Sundays and legal holidays,if immediate compliance is not possible. lf he cannot comply with the request <br /> within that time.he shall so infonn the individual,and may have an additional five days wi[hitt which co comply tvith dte request,escluding Sahirdays, <br /> Sundays and legat holidays. <br /> Subd.4.
