� � r Sec.13.0:RIGF3TS OF SUBJECTS OF DaTA Subd. I. Type oF data. The righrs of individual on whom the data is stored or[o be scored shall be as set forth in this secaon. Subd.2. Information reqtrired to be given individual. An individual asked to supply privace or confidearial data conceming himself shall be informed of: (a)the purpose and incended use of the r�quesced dara within the collecdng"stace agency, polirical subdivision,or sticewide system; (b)whecher he may refuse or is legally required to supply the requested data;(c)any lrnown coasequence arising from his supp(ying or refusing to supply � privace or confidenrial dara;and(d)the idendry of o�her persoas or enrides auchorized by state or federal law to receive[he data. 'Ihis requiremene shall not apply when an individual is asked co suppty invesrieaave dara, pursuanc to secdon 13.82, subdivision 5, co a law enforcemeac ofr;cer. - The commissioner of revenue mav place th� noace r�auired under[his subdivision in the individual income tax or proeem tax refund inscrucdons inscead of on[hose fotrns. Subd. 3. Access to data by indirldual. Upon request to a responsible auchoriry,an individual shall be informed whether he is the subject of scored data on individuals,and whecher i�is classified as public, privace or confidenrial. Upon Fus fur[her request, an individual who is the subjecc of scor.d private or public daci on individuals shall be shown che data wi�hou[any charge to him and,�if he desires, shall be informed of the concenc and meaning of chat data. Aher an individual has been shown[he priva�e data and informed of its meaning, the data need not be disclosed to him for six monrhs�hereafter unless a dispute or acaon pursuan�to chis secdon is ptnding or addirioaal data on the individual has been collected or creaced. The responsible authoriry shall provide copies of the privace or public data upon requesc by the individual subjecc of the data. The responsible authoriry may require[he requesring person to pay the accual costs of making,cerrifying,and compiling the copies. The responsible authoriry shall comply immedia�ely,if possible, wich any request made pursuant to this subdivision,or wirhin five days of the da[e of the request,excludin;Sacurdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply wi[h the request within chac time,he shall so inform the individual,and may have an addirional five days wi[hin which to compty wi[h the requesc,exctuding Saturdays, Sundays and legal holidays. Subd.4. Procedure when data is not accurate or compiete. An individual may contest the accumcy or completeness of public or private data cor.cerning himself. To exercise chis rieht,an individual shall noa"ry in wri[ing the responsible authoriry describing the nacure of the disagreement. The responsible au�horiry shall within 30 days either: (a)correct the data found to be inaccurace or incomplece and attempc to nodfy pasc recipienu of inaccura�e or incomplete data, including recipiencs named by che individual; or(b)nodfy[he individual that he believes the data to be coaect. Data in dispu[e shall be disclosed only if the individual's statement of disagreement is includzd with the disclosed data. The de[erminarion of the rzsponsible authoriry may be appealtd pursuant[o the provisions of the adminiscraave procedure act reladng to con[ested cases.
