Sec.13.04 RIGFiTS OF SUBJECTS OF DATA Subd. 1. Type of data. "Ihe righ�s of individual on whom the data is stored or to be stored shall be as set fonh in this secrion. Subd.2. Information reqirired to be given individual. �.n individual azked to supply private or confidenaal data conceming himself shall be informed of: (a)the purpose and incended use of che requesred data within the collecdng'state agency, polirical subdivision,or sratewide system; (b)whether he may refuse oY is legally required to supply the cequested data;(c)any lrnown consequence arising fmm his supplying or refusing to supply priva[e or confidendal data;and(d)the idenary of ocher penoas or enrides au[horized by srate or federal law to receive the data. This requiremenc shall no[apply when an individual is asked to supply invesrigadve dara, pursuant to section 13.82, subdivision 5, to a law enforcement o�cer. The commissioner of revenue mav place the norice rewired under this subdivision in the individual income tax or Dropem tax refund instructions instead of on those forms. Subd. 3. Access to data by individual. Upon requesc to a responsible authority,an individual shall be informed whether he is the subject of stored dara on individuals,and whe[her ic is classified as public, private or confidenual. Upon his furthec request,an individual who is the subjecc of stored private or public data on individuals shall be shown the dara wichout any charge to him and,-if he desires, shall be informed of[he content and meaning of�hat data. Aftzr an indi�+idual has been shown che private data and informed of iu meaning,the data need not be disclosed to hun for six months thereaftec unless a dispute or acdon pursuan�to this secaon is oending or addirional data on the individual 6as been collected or crea�d. The responsible auchoriry shail provide copies of the privace or public data upon request by the individual subject of the dara. The responsible authoriry may require the requesring person to pay the actual costs of makine, certifying,and compiling the copies. The responsible au[horiry shall comply immediately, if possible, with any request made pursuant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is noc possible. If he cannot comply with the reques[ wi[hin[hat time,he shall so inform[he individuai,and may have an addiaonal five days within which to comply wi[h the request,excluding Saturdays, Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private data conceming himself. To exercise chis right,an individual shail noafy in wriring the responsible authoriry describing[he nature of the disagreement. The responsible authoriry shall within 30 days either: (a)correct the data found to be inaccurate or incomplete and attempt to norify past recipiencs of inaccurate or incomplete data, including recipients named by the individual; or(b)notify the individua!that he believes rhe data to be correcc. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The decerminarion of the rzsponsib(e auchoriry may be appealed pursuant to [he provisions of the administradve procedure act relacing to contested cases. You may refuse to supply data, but refusal may require that the City deny the pemut or license. 3. The information may be shared wi[h ocher local, state or federal a;encies to the estent necessary to process the pernut or license. 4.
