� . � . . Sec.13.04 RIGHTS OF SUBJECTS OF DaT?, Subd. 1. Type of data. The rights of individual on whom the data is stored or to be scored shall be as set forth in this secdon. Subd.2. Information reqtrired to be given individual. An individual asked to supply private or confidenrial dara concerning himself shall be informed of: (a) the purpose and in[Ended use of the requesred dara within[he collecang'state ageocy, political subdivision,or sracewide rystem; (b)whether he may refuse oY is legally required to supply the requested data;(c)any lmown coasequence arising from his supplying or refusing to supply • priva[e or conndenrial data;and(d)the idenary of other persons or enrides au�horized by state or federal iaw to receive che data. Subd. 3. Access to data by individual. Upon reques[to a responsible authoriry,an individua!shall be informed whe[her he is the subject of scored data on individuals,and wherher ic is classified as public, privace or confidendal. Upon his further request,an individual who is the subjecc of stored private or public data on individuals shall be shown the data wirhout any charge to him and, if he desires, shall be informed of the contenc and meaning of chat data. Afrer an individual has been shown the privace data and informed of its meaning, the dara need not be disclosed to him for six mon�hs[hereafter unless a dispute or acdon pursuant to rhis secrion is pending or addidonal data on the individual has been collected or created. The responsible authoriry shall provide copies of the privace or public data upon requesc by the individual subject of[he data. The responsible authoriry may require the requesring person to pay the accual coscs of making,certifying, and compiling the eopies. The responsib(e authoriry shall comply immedia[ely, if possibtz, wich any request made pursuant to this subdivision, or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request within[hat time,he shall so inform che individual,and may have an addidonal five days wi[hin wtuch to comply with the request,excluding Saturdays, Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or pri�•ate data concerning himself. To exercise chis right,an individual shall noafy in wriring che responsible auchority describing the nacure of the disagreement. The responsible authoriry shall within 30 days eicher: (a)correcc the data found to be inaccurate or incomplece and attempe to noafy past recipients of inaccurate or incomplete data, including recipients named by the individual; or(b)notify ehe individual that he believes che data to be correct. Data in dispu[e shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The determinarion of the responsible authority may be appealed pursuanc to the provisions of the administrarive procedure act relaang to contested cases. DATA PRIVACY ADVISORY In accordance with M.S. 13.04, Subd.2, "Ri;hts of subjects of data", we would like to inform you that your request for a pernu[ or license from the Ciry of Orono or any of its deparcments may require you to fumish certain private or confidential information. You are notified that: 1. The information you furnish will be used to determine your qualification for the permit or license requested. 2.
