� T Sec.13.04 RIGHTS OF SITBJECTS OF DAT?, Subd. 1. Type of data. "Ihe riehcs oF ir.dividual on whom the data is stored or to bz stored shall be as set forth in this secdon. Subd.2. Informaaon reqirired to be given individual. An individual azked to supply private or confideadal dara conceming himsetf shall be informed of: (a) the purpose and intended use of the requested data wichin the collecang"state agency, polidcal subdivisioa,or stacewide system; (b)whecher he may refuse or is legally required to supply che requested data;(c)any lrnown coasequence arising from his supplying or refusing to supply • privace orconfidenrial dam;and(d)the idenary of ocher persoas or enddes auchorized by state or federai law to receive the data. Subd. 3. Access to data by indi�-idual. Upon requesc to a responsible au[horiry,an individual shall be informed whether he is the subject of scored data on individuals,and whecber ic is classified as public, private or conFidenrial. Upon his fur�her request, an individual who is che subjecc of stored private or public data on individuals shall be shown che dara wirhouc any charge to him and, if he desires, shall be informed of che concenc and meaning of�hac data. Afrer an individual has been shown ehe priva�data and informed of iu meaning, [he data need not be disclosed to him for six monrhs[hereaiter uriless a dispute or acaon pursuant to chis secaon is ptnding or addirional data on che individual has been collecced or crea[ed. The responsible auchoriry shall provide copies of the privace or public data upon request by the individual subjecc of the data. The responsible authoriry may require[he requesang person to pay the accua!costs of making,cerafying, and compiling the copies. The responsible auchoriry shall comply immediately, if possible, wich any requesc made pursuant to[his subdivision, or wichin five days of the date of the reques[,excluding Saturdays,Sundays and legal holidays,if immzdiate compliance is not possible. If he cannot comply with the request within chac time,he shall so inform�he individual,and may have an addidonal five days wiehin which[o comply with the requesc,exctuding Saturdays, Sundays and legai holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accutacy or completeness of public or private data concerning himself. To exercise chis rieh�,an individuai shall noafy in wricing che responsible authoriry describing the nanire of the disagreemenc. The responsible authoriry shall within 30 days either: (a)correcc[he da�a found to be inaccurate or incomplece and attempt co noafy pasc recipients of inaccurate or incomplete data, inetuding recipien�s named by [be individual; or(b)nodfy the individual that he believes [he data to be correct. Data in dispute shall be disclosed only if the ir.dividual's sta�zment of disagreement is includtd with the disclosed data. The determinarion of the responsibte aurhoriry may be appealzd pursuanc to the provisions of[he adminiscrarive procedute act reladng to contested cases. DATA PRIVACY ADVISORY In accordance with M.S. 13.04, Subd.2, "Rights of subjects of data", we would like to inform you that your request for a permit or license from the Ciry of Orono or any of its departments may require you to furnish certain p:ivate or confidential information. You are notified that: 1. The information you furnish will be used to deternune your qualification for the permit or license requested. 2. You may refuse to supply data, buc refusal may require that the City deny the permit or license. 3. The information may be shared wich other local, state or federal agencies to the extent necessary to process the permit or license. 4. If your requested permit or license requires Council action to approve, some information may become public. 5.
