Injormation required to be given individuaL An individ:ial asked to supply private or confidentral data concerning himself shall be informed of.• (a)the p:�rpose and intended use of the requesled data within the collecting state agency,politrca!subdivision,or statetivide system;(b)whether he may refuse or is legally requrred to sz�pply the requested data;(c)any knolvn consequence arising from his supplying or refusing to supplyprivate ar confidential data;and(dJ the rdentity of other persons or entities auihorized by state orfederal law to receive the data. This requirement shall not apply when an indivrdual is asked to supply investigative data,pursuant to section 13.82, s:�bdivision 5, to a Imv enforcemen!officer. The commrssioner of revem�e may place the notice required under this subdivisron in the individual income tac or property tax re(und instructions instead of on those jorms. Subd.3. Access to data by individual. Upon request to a responsible authorrry,an individual shal!be injormed whether he rs the subject of stored data on individuals, and whelher it is classified as publrc,private or confidential. Upon his further reguesf,an individual who is the subject of stored private or p:�blic data on individuals shall be shotivn the data withot�t any charge to hrm and, if he desrres,shall be informed of the content and meaning of that data. After an rndivrdua!has been shoivn the private data and informed of rts meaning, the data need not be disclosed to him for si.r months thereafter unless a dispule or action pursuant to this sec[ron is pending or additional data on the individua!has been collected or created. The responsible authoriry shall provide copres of the private or public data upon request by the individua!subject of the data. The responsible authority may require the reguestrng person to pay the actual costs ojmaking,cert�ing,and compiling the copies. The responsible authoriry shall camply immediately,rfpossible,tivith any request made purst�ant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays,rf im.mediate compliance is not possible. If he cannot comply lvith the request within that time,he sha!/so inform the individual,and may have an additional five days tivithrn which to comply with the request,excluding Saturdays,Sundays and legal holydays. Subd.4. Procedure when data is not accurate or comp(ete. An individual may contest the accuracy or completeness ofpubl ic or private data concerning himself. To exercise this right, an indivrdual shall not�in writing the responsible authority describing the nature of the disagreemenl. The responsible authority shall within 30 days erther: (aJ correct the datafound to be inaccurate or rncomplete and attempt to notify past recipients oJrnaccurate or incomplete data,including recipients named by the individual;or(bJ not�the indrvidual that he believes the data to be correct. Data in drspute shall be drsclosed only rf the individua!'s slatement of disagreement is included tivith the disclosed data. The detenninatron of the responsible authoriry may be appealed purs:iant to the provrsions of the administrative procedure act relating to contested cases.
