A brief description of the incident. <br />Include events which led to the use of the <br />stun gun and the results. <br />c. State if stun gun was or was not effective. <br />d. Include sex, age, physical description of <br />subject. <br />C. Report required. (Special Order 16.5) <br />During the field testing period a report must be made on <br />each incident involving officer use or threat of use of <br />the stun gun. <br />1. Report shall include: <br />a. Date, time and location. <br />b. Description of suspect. Include sex, age, <br />physical description- note if he/she has <br />history of heart problems. <br />c.
