Additional information on the <br />responsibilities outlined below can be found in the attached information packets. <br />Forms to be filled out and submitted with your bid are contained in these packets. <br />LABOR STANDARDS <br />1. Compliance with HUD 4010 Federal Labor Standards Provisions on all <br />Community Development funded projects of 52,000 or more. <br />.2. Compliance with Prevailino Wage Requirements. <br />3. Submittal of completed Contractor Certification HUD 1421 and <br />Subcontractor Certification HUD 1422 alona w-_th bid. <br />.•� 4. Submittal of signed Section 3 Compliance Certificate along with bid. <br />EQUAL EMPLOYMENT OPPORTUNITY <br />1. Compliance with Equal Employment Opportunity (EEO) provisions of <br />Executive Order 11246 on all contracts of 510,000 or more. <br />-- 2. Submittal of com letedEqual Employment Opportunity Compliance Certificate <br />by prime contractors and subcontractors along with bid. <br />3. Submittal of Construction Contractor Identification -Data form along <br />with bid. <br />- 4. Employment goals and timetables for women and minorities. <br />5. Completion of Monthly Employment Utilization Report. <br />',. 6. Orr contracts for $50.000 or more and you have more than 50 non -construction <br />employees you must have a written (AAP) Affirmative Action Pltn. To be <br />submitted with bid. <br />7.
