2288.2 <br />CITY OF ORONO <br />AGENT OF RECORD AGREEMENT <br />Duweyn P. Carlson of the Apple Valley Agency agrees to perform <br />the following services for the City of Orono: <br />1. The negotiation and placement of all the required <br />insurance; <br />2. Information on relevant changes in insurance <br />markets, products and services; <br />3. Assistance in developing comprehensive insurance <br />coverages; <br />4. Provide an insurance policy inventory and summary; <br />5. A complete accounting of commissions earned (direct <br />only) on the account; <br />6. A cumulative five-year premium and loss record; <br />7. Prepare an allocation of premium by department as <br />required by the City; <br />8. Loss exposure list and classification; up -dated <br />annually; <br />9. Assist with all claims and losses; <br />10. Assistance in minimizing premiums; reviewing and <br />auditing rating basis, formulas, etc. <br />11. Assistance in loss prevention engineering and loss <br />control; <br />12. willingness to keep an in -agency record of losses <br />including reserves established; <br />13. Assist the City in Risk Management program. <br />The fee for service for the Insurance Policy year 1988 will be <br />$5,500.00 and will cover the following: <br />1. Conduct an Exposure survey and prepare <br />specifications <br />2. Obtain quotes <br />3. Review policies, hillings and claims <br />
