To: File <br />Frcu: Stephen Weckman, On -Site Systems Manager <br />Date: October 21, 1991 <br />Subject: Septic System - 315 Crestview Avenue <br />Since the septic inspection program was implemented in 1978, <br />the system has been inspected in 1978, 1984, 1986, 1987, 1990 and <br />1991 according to our septic system inventory card. Generally <br />for each inspection, a report form is completed and a copy is <br />sent to the homeowner. At this time, however, reports for the <br />inspections made in 1986 and 1987 are not in the City address <br />file. <br />The septic system was observed discharging effluent to the <br />surface on December 3, 1987 according to the septic system <br />inventory card. As there is no report on file for the <br />inspection, it is doubtful that a report was sent to the <br />homeowner at that time. It is likely that a report was never <br />completed as the inspection was performed in the winter when a <br />thorough inspection may not have been possible. <br />In June of 1991, the septic system was observed to be <br />discharging effluent to the surface in the neighbor's yard and <br />repairs were required. Mr. Shannon came in to the City offices <br />in early June to discuss his septic system and the repairs <br />required. At that time he told me he was unaware that there were <br />any previous problems with the septic system. <br />We don't have any way to verify that the previous owner, Jim <br />Buelow, knew that the septic system was failing while he owned <br />the property. <br />Enclosure: Septic System Inventory Card - 315 Crestview Avenue <br />lsv <br />
