Completed Application Form <br />Certified Froperty Owners List of o%mers within 150' (you must obtain <br />this list from Hennepin County Department of Finance A-603 Govt Center <br />348-3271). <br />Stamped, legal sized envelopes (110) pre-addressed to each of the <br />names on the above list with no return address (use address labels <br />obtained witJi property owners list). <br />Certificate of survey including hardcover calculations as required. <br />Topographic survey (existing and proposed eleyatiox^s) if any changes <br />in existing grade are proposed. <br />Flat Map (obtained with property owners list). <br />As an addendum to this application, please attach a separate list of <br />any other persons you wish notified of this application. <br />8.
