PROPOSAL DOCUMEHTS <br />All proposals submitted must include all of the below listed <br />documents. Failure to do so may result in rejection of the <br />entire proposal. Please file in the order listed. <br />1. Completed and signed proposal and specifications on <br />forms provided. (Proposal requirements are a part of the <br />proposal package.) <br />2. List of deviations and exceptions (if any) with <br />appropriate explanation. <br />3. System environment description statement. <br />4. Listing of equipment to be installed. <br />5. Listing of current installations in Metropolitan area <br />■ which utilize projosed system. List of installers ana <br />similar installations they have worked to install. <br />6. Copy of proposed warranty/guarantee agreement. <br />7. Copy of proposed service contract. <br />8. Descriptive literature on system together with proposed <br />training progreun and user instruments. <br />9. Listing of names of personnel certified for maintenance <br />as outline in specifications. <br />10. Certification from the manufacturer that they <br />provide service and parts for the period as outlined in the <br />bid specifications. <br />11. Listing of expansion costs. <br />12. Alternative bid proposals. <br />NOTH: City retains option to <br />information about both contractor and manufacturer, <br />certified audit for the last two (2) years showing assets and net <br />worth and attesting to the financial stability of each. <br />Page 14 of 17
