AFMN Community Grant Application with Assurances (GK203)34 <br />A complete application will include the following, depending on the entity type and request <br />amount: <br />For Applicants who are units of government or for-profit businesses: <br />☐ Completed and signed coversheet, assurances, and application. (Note: By <br />submitting the signed cover sheet, you are agreeing to the Assurances in Section 2.B of this <br />application.) Please send only your completed Cover Sheet and Section 3: Grant Application <br />when you submit your application.) <br />☐ Additional budget document (optional) <br />For Applicants who are Non-Governmental Organizations (NGOs)* requesting less <br />than $50,000: <br />(*NGOs are organizations independent of government and business—e.g., nonprofit <br />organizations) <br />☐ Completed and signed coversheet, assurances, and application.
