AFMN Community Grant Application with Assurances (GK203)11 <br />budget, timelines, and other supplemental information submitted in support of the approved Grant <br />Application and Assurances and in accordance with the terms identified in the Official Grant Award <br />Notice. <br />Costs associated with preparing this application must be borne by applicants. <br />The burden of proof of timely submission is on the applicant. <br />4. Public Data <br />Per Minnesota Statutes, section 13.599: <br />Names and addresses of Grant Applicants will be public data once application materials are opened. All <br />remaining data in proposal responses (except trade secret data as defined and classified in Minn. Stat. <br />§ 13.37) will be public data after the evaluation process is completed (for the purposes of this grant <br />application, when all Official Grant Award Notices have been issued by the state agency to the <br />Grantees). All data created or maintained by State as part of the evaluation process (except trade <br />secret data as defined and classified in Minn. Stat.§13.37) will be public data after the evaluation <br />process is completed (for the purposes of this grant, when all Official Grant Award Notices have been <br />issued by the State to the Grantees). <br />H. Application Screening and Review Process <br />Phase 1: Screening <br />Applications that meet the following criteria will be forwarded for further consideration and review. <br />1.Application is received via email by Friday, November 8, 2024 at 4:00 p.m. Central <br />time. <br />2.The Grant Applicant meets the minimum eligibility of the grant. (See Section 1 under Eligibility <br />Criteria for details.) <br />3.Application is complete. (See last page of this application for details.) <br />Phase 2: Application Components Scored <br />Each Grant Application and Assurances will be reviewed and scored as indicated below.
