Grant Application Components <br />1. Coversheet <br />a. All Grant Applicants must include a completed Cover Sheet that includes all requested <br />information. Including: <br />A. An original signature from the identified official with authority to sign on behalf of <br />the Grant Applicant. <br />B. Contact information for the agency, agency head, the program contact and the <br />accounting or business office manager. <br />b. Identification Numbers, if applicable, are requested as part of the Cover Sheet. <br />Your organization may already have these number(s), check with your business office. <br />Minnesota Statewide Integrated Financial Tool System (SWIFT) Vendor <br />Number: Please reference this information on SWIFT: <br />https://mn.gov/mmb/accounting/swift/vendor-resources/ <br />In the application section, develop your responses to each component, both the narrative portions and <br />the table. Each criterion within the application narrative sections should be addressed in the narrative <br />response. Space is provided to answer each narrative section. <br />The total page count recommended for the narrative components is: 2-3 pages per funding area, <br />based on word count. <br />Total points possible for each application is: 100. <br />The completed application should consist of the following: <br />Completed and signed Application and Assurances <br />Grant Application Narrative Section <br />Budget narrative using template <br />2. Applicant Questions <br />Applicants will have opportunities to ask questions related to the applications. <br />a. A Virtual Grant Applicant Conference will serve as an opportunity for Applicants to ask <br />specific questions of State staff concerning the project. Attendance at the Grant Applicant <br />Conference is not mandatory but is recommended. <br />o Oral answers given at the Grant Applicant Conference will be non-binding. Written <br />responses to questions asked at the conference will be sent to all identified prospective <br />applicants after the conference. <br />102
