When <br />completing the form, the supervisor should use specific <br />examples of conduct to formulate comments. <br /> <br /> Evaluation <br /> <br /> <br />Goal <br />Setting <br />Meeting <br />The supervisor and employee will meet to discuss the current <br />evaluation and set goals for the coming rating period. They <br />should meet in an atmosphere that is both private and free of <br />interruptions, and the supervisor should take particular care to <br />make the meeting a priority. There should be an open and <br />honest exchange where each is permitted to state his or her <br />opinion regarding the answer to any section. <br />January <br />15-31 <br /> <br /> <br /> <br /> <br />Department <br />Director <br />Review <br />After the meeting, the supervisor will finalize the evaluation <br />form including any additional comments. If an employee <br />disagrees with a particular answer, he or she may note the <br />disagreement on the form or by attaching a separate <br />document. When the form has been completed, it will be <br />signed by the employee, the supervisor, and the department <br />director. The supervisor will provide the original copy to the <br />City Management Assistant, will keep a copy in his or her <br />files, and will provide a copy to the employee. <br />NLT 31 January <br /> <br /> <br />4. Responsibilities: <br />A. City Administrator. The City Administrator is responsible for establishing the <br />performance evaluation policy and process. <br /> <br />B. Department Director. Department heads are responsible for ensuring the <br />evaluation process is completed within their departments on time and to standard. <br />Department Heads will review all evaluation within their departments to ensure they are <br />complete, consistent, and fair. <br /> <br />C. Supervisor. Supervisors are responsible to coordinating the completion of the <br />evaluation and goal setting process with their subordinate employees and ensure the <br />process is documented and recorded in the employee’s personnel files. Supervisors are <br />responsible for ensuring all of their employees receive a fair and objective evaluation. <br />Supervisors are responsible for scheduling and leading ongoing check-ins with their <br />subordinates. <br /> <br />D. Employee. Employees are responsible for preparing the self-evaluation portions <br />of the evaluation process and working with their supervisors to establish goals. <br /> <br />E. Management Assistant. The Management Assistant shall ensure personnel files <br />are kept up to date and performance evaluations are filed appropriately. The <br />133
