s <br />-10-10 J <br />G.a list of reportable communicable diseases as defined in part <br />4605.7040; <br />H. <br />I. <br />the separation reports mandated in part 9503.0055; <br />the report by the health consultant mandated in part 9503.0140; <br />and <br />J.a record for each consultant whose services the center purchases <br />either by contracting directly with the individual or by <br />contracting for the person's services with another organization. <br />The record must contain a copy of a signed contract or letter of <br />appointment specifying conditions and terms of employment and <br />documentation that the person under contract meets any licensure, <br />registration, or certification requirements required to perform <br />the service specified in the contract. <br />9503.0025 APPLICANT BACKGROUND STUDY. <br />Subpart 2. Information on organizations. If the applicant is an <br />authorized representative of a partnership, corporation, volxmtary <br />association, or other organization legally responsible for the operation of <br />the child care program, the applicant must make available and maintain the <br />names and addresses of the owners and board members, the articles of <br />incorporation, and an organizational chart in the center administrative record <br />specified in part 9503.0115. <br />i
