FBOM Id.20. 1993 loiie ?. <br />WAIVER <br />ACKNOWLEDGMENT AND ASSUMPTION OF RISK AND RELEASE <br />I,____________________________, know that nordic skiing is an action sport <br />carrying significant risk of serious injury, death or property damage. 1 also know that are <br />natural, mechanical and environmental conditions and risks which independently or in <br />combination with my activities may cause property damage or severe or even fatal injuries <br />to me or others. <br />I agree that I am alone responsible for my safety while training and participating in nordic <br />ski events and I specifically acknowledge that the following persons or entities including <br />Peter Welles, Jan Guenther, Gear West Ski & Bike, the City of Orono and any agent, <br />representative, officer, director, employee, member or any affiliate of any person or entity <br />named above are not responsible for my safety. <br />I specifically RELEASE and DISCHARGE, in advance, those parties from any and all <br />liability whether known or unknown, even that liability may arise out of carelessness on <br />the part of persons or entities mentioned above. I agree to accept all responsibility for the <br />risks, conditions and hazards which may occur whether they be known or unknown. <br />Being fully aware of the risks, conditions and hazards of nordic skiing, I HEREBY <br />AGREE TO WAIVE, RELEASE AND DISCHARGE any and all claims for damages <br />death, personal injury or property damage which I may have as a result of my participation <br />in training or participation in nordic ski events, against any person or entity named above <br />whether such injury or damage was foreseeable or not. <br />1 further agree to forever HOLD HARMLESS and INDEMNIFY all persons and entities <br />identified above, generally and specifically, from any and all liability for death, personal <br />injury or property damage, resulting in any way ft^om my training or participating in nordic <br />ski activities. <br />This Acknowledgment and Assumption of Risk and Release shall be binding upon my <br />heirs and assigns. <br />Signature:.Date: <br />Pai.nl's Signature (if participant is under 18):,
