Rd. <br />Wayzata, MN 55391 <br />Dear Mr. Ulrich: <br />PERMIT 86-6178, ONE ^VELL, HH^INEPIN COUNTY <br />Enclosed is Permit 86-6173 which permits the appropriation of water for <br />the purpose of heating and cooling on a continucxis basis in Section 8, <br />T117N, R32W. <br />Please read all the conditions of the permit, especially Condition 3.b. This <br />requires you to record the amounts of water appropriated. A Water Use Report <br />will be sent to you each January for reporting the amounts for the previous <br />year. The report must be submitted with the processing fee by February 15 of <br />each year. The fee schedule is $5.00 per each permitted ten million gallons or <br />fraction thereof. The permit authorizes the appropriation of 11 million <br />gallons per year; therefore, the annual processing fee is$15.0Q. Do not send the <br />report and fee until notified. Failure to submit the report and fee can result <br />in the termination of the permit. <br />Pursuant to Minnesota Statutes, Section 105.44. Subds.3 and 6, the applicant, <br />the managers of the watershed district, the board of supervisors of the soil <br />and water conservation district, or the mayor of the city may demand a hearing on <br />the Permit provided the demand for hearing and the bond required are filed with <br />the Commissioner within 30 days of receipt. A corporate surety bond or <br />equivalent security in the amount of $500.00 must accompany the demand for <br />hearing; however, the applicant may be responsible for hearing costs up to <br />$750.00. No bond is required of a public authority which demands a hearing. If <br />no demand for hearing is made or if a hearing is demanded but no bond is filed, <br />the Permit shall become final at the expiration of the 30 days and no appeal may <br />be taken to the State Court of Appeals.
