3. The facility shall be overseen by a Board of Directors. The <br />applicant shall submit bylaws of the Board of Directors, resumes <br />of members of the Board of Directors and Articles of <br />Incorporation to the City. <br />4. A committee shall be established consisting of the Orono Police <br />Chief, an Orono City Councilmember, a neighborhood <br />representative, a representative of the applicant organization, and <br />a fifth member to be agreed upon by the City and the applicant. <br />The committee’s purpose shall be to review the openrions and <br />complaints, if any, associated with the Shelter, and upon an annual <br />basis, to report its findings to the Orono City Council for <br />determination as to compliance with the terms of the conditional <br />use permit as well as other State and local ordinances, stanites and <br />regulations. <br />5. In the event that upon such a review by the Committee, the Orono <br />City Council finds that the terms of the conditional use permit are <br />not being substantially complied with or that State and local <br />ordinances, statutes or regulations are being violated, the <br />conditional use permit shall be revoked. <br />6. A background investigation shall be performed to verify the <br />MW iilicr.tion of tlie Director/Administrator and the responsible <br />operation of the program, such investigation to be paid for by the <br />applicant. <br />7. Per City ordinance requirement, the conditional use permit if <br />granted will be issued for the shelter use for a specific site and not <br />to a specific applicant, however, any material change in the <br />operations or purpose of facility shall be cause for the City <br />Council to review the conditional use permit. <br />IV. Additional conditions may be required by the City in order to address the <br />specific impacts of a proposed shelter." <br />SECTION 2. Adoption and Publication.
