3. <br />4. <br />5. <br />The applicant shall submit detailed program information including <br />goals, policies, activities schedule, staffing patterns and targeted <br />capacity which may result in the imposition of reasonable <br />conditions to limit the off-site impacts. <br />The facility shall be overseen by a Board of Directors. The <br />applicant shall submit Bylaws of the Board, resumes of the <br />Boardmembers. and Articles of Incorporation to the City. <br />The City Council may annually appoint a representative to the <br />Board of Directors, which representative .hall act as a liaison <br />between the Board and the Council. <br />Per City ordinance requirement, the conditional use permit if <br />granted will be issued for the shelter use for a specific site and not <br />to a specific applicant. Any material change in the operations or <br />purpose of the facility shall be cause for the City Council to <br />review the conditional use permit. Further, the City Council may <br />review the conditional use permit if Council is presented with <br />evidence that the terms of the conditional use permit as well as <br />other State and local ordinances, statutes and regulations are being <br />violated. <br />IV. Additional conditions may be required by the City in order to address the <br />specific impacts of a proposed shelter." <br />SECTION 2. Adoption and Publication.
