DEFINITIONS OP MONTHLY REPORT ITEMS <br />fiPHOQL ACTIVITIES <br />CLIENT INITIATED: Means that either the student, parent or faculty contacted <br />me. (Depending on which category is listed) <br />POLICE INITIATED: These are contacts that I initiate. <br />MEETINGS ATTENDED: Number of meetings that I attend. (e.g.. Student <br />Assistance Team meetings, Liaison review meetings, etc.) <br />PROFESSIONAL CONTACTS: Includes Social Workers, Psychologists, Lawyers, <br />Special Education Teachers, Police Officers from other departments. I count a <br />professional contact as a contact where a specific request is being made. I <br />do not count this as contact if it is simply an exchange of information. <br />STUDENT CONTACT: Significant contact with a student. This usually means that <br />I have talked to them face to face about a specific problem or situation. <br />COUNSELING: Any matter tha is not related to a crime or criminal <br />investigation. <br />LAW ENFORCEMENT: Any contact that has to do with a criminal case or <br />potential criminal case. <br />PARENT CONTACT: Significant contact with a parent. This usually is conducted <br />over the phone. Often it is related to a specific problem that a parent is <br />having with a child. <br />TEACHER CONTACT: Any significant contact with a teacher. <br />REQUEST FOR CLASS: If a teacher asks me to give a class or requests that <br />I find someone in law enforcement to give a class. <br />STUDENT INFO: Teacher requesting information about a specific student. <br />They usually have noticed a change in the student's behavior and would <br />like additional information. <br />MISC.: Any other information that they need/request dealing with police <br />work in general. <br />EXTRA CURRICULAR: Any activities that I attend that are sponsored by the <br />school and are normally not during school hours.
