ENVIRONMENTAL ASSESSMENT WORKSHEET (EAW) <br />NOTE TO PREPARERS . ^ <br />This worksheet is to be completed by the Resoonsible Governmental Unit (RGU) or its agents. The <br />project proposer must supply any reasonaoly accessible data necessary for the worksheet, but is not <br />to complete the final worksheet itself. If a complete answer does not fit in the space allotted, <br />attached additional sheets as necessary. <br />For assistance with this worksheet contact the Minnesota Environmental Quality Board (EQB) at <br />(612) 296*8253 or (toll free) 1-800-652-9747 (ask operator for the EQB environmental review <br />program) or consult "EAW Guidelines," a booklet available from the EQB. <br />Comments must be submitted to the RGU (see item 3) during the 30-day comment period following <br />notice of the EAW in the EQB Monitor . (Contact the RGU or EQB to learn when the comment period <br />ends ) Comments should address the accuracv and completeness of the information, potential <br />impacts that may warrant further «>,;sstigatior„ and the need for an EIS. If the EAW has been <br />prepared for the scoping of an EIS (see item 4), comments should address the accuracy and <br />completeness of the information and suggest issues for investigation in the EIS. <br />1.
